We present a case of a 17-year-old girl, called Z, who was diagnosed with Social Anxiety Disorder (SAD) and undergone CBT. Z presented with school refusal and social avoidance due to excessive negative thoughts about herself and others thinking badly of her. Methods: A brief psychoeducational-based CBT was conducted over 10 weekly sessions with the emphasis on psychoeducation, behavioural and cognitive skills training, and practices at home, as well as reflective discussion to modify negative beliefs, increase the use of positive coping strategies, and understanding of problematic experiences. Pre-intervention scores were compared to mid and post-intervention using Beck Youth Inventory (BYI) and Assimilation of Problematic Experiences Scales (APES). Outcomes: Pre-intervention BYI scores revealed an extremely elevated level of anxiety and depression symptoms, anger, and disruptive behaviours, with a much lower-than-average level of self-concept. APES revealed a score of 1/7 at the pre-intervention phase. Towards the end of the session, Z reported significantly reduced scores in anxiety, depression, and anger symptoms. The scores on the APES demonstrated an increased awareness, understanding and clarification towards her problematic experience, as well as application of her understanding in problem-solving efforts. Re-emergence is seen amongst immunocompromised individuals whose diagnosis may be overlooked, leading to unnecessary surgery. Diagnosis is challenging in the absence of systemic constitutional symptoms and depends mainly on the treating physician having a strong suspicion. We report a rare extrapulmonary manifestation of TB involving the parotid gland in an otherwise immunocompetent child. A 2-year-old girl presented with a mass in the right parotid region that had been present for 3 months. A similar swelling was treated with an antibiotic 3 months prior, but it recurred. The mass was painful, firm, erythematous, and located in the infra-auricular region. Symptoms persisted after 2 courses of antibiotics, after which ultrasonography was performed and the intra-parotid collection was aspirated, which was positive for acid-fast bacilli (AFB). The patient was placed on anti-TB treatments. It leads to liver phosphorylase deficiency, thus causing glycogenolysis disorder. Classic manifestations are mild hypoglycaemia, abdominal distension, growth retardation, hepatomegaly, elevated liver transaminases, hyperlipidaemia, and normal lactate and uric acid. This proband is a 14-year-old Malay girl who was the second child of non-consanguineous parents. At four years old, she was referred for the incidental finding of hepatomegaly with deranged liver enzymes. Clinically, there was hepatomegaly (4 cm below the right costal margin) without any growth retardation. Subsequently, at 11 and 13 years old, she experienced a twisted left ovarian cyst and acute colitis, respectively. Biochemically, there was significantly increased transaminases, hypertriglyceridaemia, and mild hypoglycaemia. The liver biopsy result was consistent with GSD. Next-gene sequence analysis test revealed compound heterozygous mutations identified on the PYGL gene: splice site c.772+2_772+3del and missense c.2071G>C (p.Gly691Arg). Biochemical parameters were normalised except for persistent hypertriglyceridemia after treatment with uncooked corn starch four times daily. Family screening of mother and younger brother exhibits both are carriers of a missense mutation at c.2071G>C(p.Gly691Arg). Since 2019, the global pandemic caused by SARS-CoV-2 infection has significantly impacted worldwide healthcare. It has variable manifestations such as respiratory and extra-respiratory manifestations, and paediatric intussusception has been reported as one of the rare gastrointestinal manifestations of COVID-19. Though COVID-19 is mainly associated with respiratory manifestations, paediatric population may present with gastrointestinal symptoms such as vomiting, diarrhoea, abdominal pain and poor oral intake, before or with the onset of respiratory symptoms. Paediatric intussusception in COVID-19 is uncommon, and the initial presentation may be mistaken not as gastrointestinal manifestation of COVID-19. It is important for physicians to have a high clinical suspicion, especially in paediatric population, to differentiate between these entities. Early recognition and prompt intervention can improve patient care and prevent complications. This case also raises the possibility of role of SARS-COV-2 virus in etiopathological cause of paediatric intussusception.
