Imbalance between anabolism and catabolism lead to degradation of articular cartilage which may further initiate inflammatory cascades. There is an interplay of mechanical and immune-mediated injuries that lead to cartilage destruction and inflammation in OA. The mainstay of OA treatment involves drugs like paracetamol and non-steroidal anti-inflammatory drugs (NSAIDs) which provide symptomatic relief in many cases; but are unable to inhibit disease progression. Also, long-term use of these drugs, is associated with major safety concerns. Native collagen type II (NC-II) and Aflapin, especially when used in combination, can slow down the disease progression in OA and serve as a safer and effective treatment option for OA. NC-II is non-hydrolyzed collagen having intact triple helix structure with active epitopes and antigenicity. Aflapin is a novel synergistic composition of Boswellia serrata gum resin having higher composition of 3-O-acetyl-11-keto-betaboswellic acid (AKBA). Various experimental studies have demonstrated the mechanisms by which both these agents exert their benefit in OA. Further, multiple clinical studies have demonstrated the efficacy and safety of NC-II and Aflapin, when used individually or as a combination. This is a narrative review of the pathophysiology of OA, current treatment modalities, and non-clinical and clinical evidence of the beneficial effects of NC-II and Aflapin in the management of OA.","PeriodicalId":487580,"journal":{"name":"International Journal of Research in Orthopaedics","volume":" 16","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141672566","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Nevertheless, the human body might have adverse effects due to traumatic incidents, degenerative conditions, or repetitive stress, resulting in the manifestation of pain, inflammation, and impaired functionality. The primary objective of this study is to assess the effectiveness of stem cell therapy in the treatment of articular cartilage damage such as lesions. The systematic review was conducted adhering to the PRISMA principles. Boolean logic is employed in the search technique to integrate phrases pertaining to stem cell therapy, articular cartilage diseases, and biological processes. The research employs a range of databases and medical subject headings (MeSH) in order to facilitate a thorough examination. The stem cell therapy in the treatment of articular cartilage lesions has great potential. It enhances management and treatment outcomes for individuals afflicted with cartilage abnormalities. It is beneficial particularly those diagnosed with osteoarthritis (OA). Encouraging outcomes, such as alleviation of pain and improvement in functionality have been reported. Additional investigation is required to enhance therapy regimens, encompassing cell dosage, delivery techniques, and supplementary medicines. The use of stem cell therapies necessitates the imperative collaboration among researchers, doctors and regulatory agencies.","PeriodicalId":487580,"journal":{"name":"International Journal of Research in Orthopaedics","volume":"37 12","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140735459","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
