Various respiratory conditions like chronic respiratory diseases or infections necessitate effective sputum expectoration because inadequate clearance can lead to complications due to retained secretions in the airways. This review discusses various pulmonary rehabilitation methods for sputum expectoration, facilitating mucus loosening/mobilization, and cough expulsion. Mucus loosening and mobilization can be achieved by postural drainage, several breathing techniques, positive expiratory pressure (PEP), oscillating PEP, and vibration methods (via external chest wall or direct airway). The latter methods include huffing, manual assist cough, mechanical in-exsufflation, and artificial glottic device. Robot-assisted gait training (RAGT) effectively improves gait, and it is primarily studied in neurological conditions, with limited research in respiratory diseases. A 68-year-old male with miliary tuberculosis and severe gait impairment received RAGT after hospitalization. Initially unable to walk, he underwent nine RAGT sessions, resulting in independent walking, improved functional parameters, and decreased dyspnea. The patient regained the ability to perform daily activities and was discharged. However, abdominal surgery methods can vary depending on the organs involved or the preferences of the surgeon, leading to diverse incision techniques. Consequently, standardizing pulmonary rehabilitation becomes challenging due to these variations. Additionally, patients exhibit varying levels of physical function, necessitating the provision of individualized services. Thus, preoperative assessment of physical function plays a crucial role, and preoperative pulmonary rehabilitation may be particularly significant for high-risk groups. While more extensive multicenter randomized studies are needed to precisely categorize subjects, based on clinical experience, pulmonary rehabilitation is not detrimental to patients and appears to contribute to improving pulmonary function and ambulatory abilities. Because inspiratory muscle dysfunction is closely related to dyspnea, which is the most salient exercise-limiting symptom of patients with chronic lung diseases, IMT has been thought to be able to reduce dyspnea and increase the functional capacity of the patients. IMT was initially examined for its effectiveness mainly on patients with chronic obstructive pulmonary disease, then its application has gradually been expanded to various disorders which could cause dyspnea. Consequently, exercise-based cardiac rehabilitation (CR) has been recommended as a treatment for patients with cardiovascular disease (CVD). Nevertheless, it remains controversial which exercise characteristics are most beneficial for patients with CVD. There is growing evidence that high-intensity interval training (HIIT) is superior to moderate-intensity continuous training (MICT) in improving CRF within the cardiac population. And HIIT appears to be as safe as MICT. However, there is no universal criteria or framework for the prescription and monitoring of HIIT in clinical populations.
