The article presents the definition of OH and the features of the course in the elderly. Particular attention is paid to the review of the results of scientific studies on the effect of OH on the risk of coronary, cerebrovascular events and death from all causes. The found literature is analyzed and summarized by the authors of this literature review. Conclusion: OH is a common condition that doctors of various specialties often encounter in practice. This disease sharply worsens the quality of life, often leads to hospitalization and often to disability of the patient. The combination of OH and supine hypertension requires more careful selection of therapy, assessment of short-term and medium-term risks. The use of simple, effective, practical measures to diagnose, monitor and alleviate this problem can have a significant impact on maintaining independence in older people.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"156 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135661818","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Currently, in developed countries, due to the development of new technologies and improvement of medical care and prevention, there is a tendency to decrease the mortality and morbidity of cerebrovascular disease (CVD). Timely and adequate use of antithrombotic therapy in patients in this category for primary and secondary prevention of ACV is an extremely important aspect of management and reliably reduces the risks of recurrent cerebrovascular accidents, increases survival and contributes to a more favorable prognosis for the recovery of impaired functions and quality of life in the long term.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"24 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135663108","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The need for early detection of CM determines the importance and relevance of informing dermatologists, cosmetologists, doctors of other specialties about the current aspects of the diagnosis and treatment of this disease, despite its relatively rare occurrence. The review highlights modern approaches to the diagnosis and treatment of CM. Early treatment of CM is vital because the disease progresses extremely rapidly with the formation of distant metastases. For the treatment of CM at localized stages, positive results in terms of prognosis in patients are brought by the use of the standard of combined treatment: surgery with adjuvant radiation therapy. A good alternative is also to use of RT only. Effective therapy for late variants of CM includes the appointment of immune response checkpoint inhibitors. Immunotherapy provides a sustained response in a significant proportion of patients with metastatic or recurrent disease. However, in the population of elderly and debilitated patients, the use of immunotherapy is limited due to the development of adverse events. The tasks of determining the optimal therapy regimens with the best efficacy and drug safety profiles have been included in the programs for further clinical research and development.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"38 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135663092","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In most cases, the appearance of a bronchial fistula is associated with the progression of the oncological process, post-radiation changes in the bronchus stump, and also with a persistent pulmonary infection. The incidence of bronchus suture failure after pneumonectomy for non-small cell lung cancer is 1.5-12.5%, after lobectomy – 1-4%. Among the predictors of the development of this complication, there are: male sex, age over 65 years, patients with severe (ASA 3 or higher) and numerous concomitant diseases, right-sided and extended operations, combined antitumor treatment. A special role in the prevention of the development of bronchopleural fistula is given to surgical techniques for covering the bronchus stump with various tissues, mainly the pleura or muscles. The treatment of bronchial fistula involves the relief of the general inflammatory reaction, the sanitation of the residual pleural cavity and the direct closure of the defect, using various options for broncho- and thoracoplasty.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135663189","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A modern classification of AML is presented. It has been shown that in patients with AML, the addition of coronavirus infection SARS-CoV-2 COVID-19 significantly worsens the prognosis of the patient’s life, which makes significant changes in overall survival and mortality. Factors worsening the prognosis of life in patients with AML are advanced age, comorbidities, AML out of remission, the critical course of SARS-CoV-2 COVID19, interruption of treatment without subsequent resumption. In patients with AML and SARS-CoV-2 COVID19, the most optimal treatment tactic is temporary discontinuation of the course of chemotherapy followed by resumption after the regression of the viral infection (both at the onset of the disease and when chemotherapy is already underway). If it is not possible to postpone the course of chemotherapy, treatment can and should be carried out with the provision of adequate accompanying therapy simultaneously with ethological and pathogenetic therapy for SARS-CoV-2 COVID19. Even in the presence of hematological contraindications in patients with AML, SARS-CoV-2 COVID19 treatment should be carried out in full according to clinical guidelines for the treatment of SARS-CoV-2 COVID19.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"70 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135663184","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In a number of industrialized countries, LC is the dominant oncological disease. The beginning of autumn 2022 was marked by a regular landmark event in the professional environment of oncologists: the Annual Congress of the European Society for Medical Oncology (ESMO) was held in Paris. The congress presented and discussed innovative developments in immunotherapy for patients with advanced NSCLC. The purpose of this work was to summarize the current clinical developments in combined immunotherapy for advanced NSCLC discussed at the ESMO Annual Congress – September 2022: IFCT-1701 studies; KEYNOTE-407; KEYNOTE-189; IPSOS; INCREASE. The systematic search for effective antitumor immunodrugs continues, and a comprehensive modernization of the protocols for their use is being carried out. This allows us to hope for a cardinal increase in life expectancy and improvement in its quality in patients with NSCLC, even at the most advanced stages.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135661814","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Age and the presence of concomitant diseases are not only risk factors for thrombosis, but also increase the likelihood of bleeding. This significantly complicates the problem of choosing anticoagulant therapy. Thus, chronic heart failure, chronic obstructive pulmonary disease, diabetes mellitus, cancer, etc. are established prothrombotic risk factors. Acquired hemophilia and von Willebrand syndrome, factor XI or vitamin K deficiency, thrombocytopenia and thrombocytopathy, on the contrary, may be risk factors for bleeding. Additional difficulties in prescribing anticoagulant therapy to elderly patients arise due to common problems in old age: renal failure, impaired liver function, polypharmacy, etc. However, the presence of these features is not a reason to refuse anticoagulant therapy, since the risk of thrombosis in such patients, as a rule, exceeds the risk of bleeding. This review details the problems that a clinician may face when prescribing anticoagulant therapy to elderly patients and provides recommendations for overcoming them. It is also reported that in the absence of contraindications, direct oral anticoagulants are the best option for the treatment of venous thromboembolic complications.","PeriodicalId":472385,"journal":{"name":"A I Burnasyan Federal Medical Biophysical Center Clinical Bulletin","volume":"205 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135663105","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
