Several studies have investigated the short and long-term complications of bariatric surgery but there is little available data on the longterm consequences on bone metabolism and the consequences of this after bariatric surgery. The purpose of this paper is to present a case of a tumoral calcinosis with uncommon characteristics in order to discuss the diagnosis and treatment options. \u0000 \u0000Case report: We report the case of a 65 years-old patient with a multifactorial avascular necrosis of the femoral head, to which a hip replacement was planned. Due to the images observed preoperatively, infection was ruled out and after the studies; an isolated tumoral calcinosis was identified and confirmed with histopathological findings. \u0000 \u0000Conclusion: Tumoral calcinosis is a benign pathology that can create confusion due to the nonspecific characteristics. Metabolic profile studies and multimodal imaging are recommended to diagnose this condition adequately. They range in size from 50 nm to 5,000 nm and, based on their size, are sub-classified into exosomes, microvesicles and apoptotic bodies. While the content and formation of EV are diverse, research has yet to pinpoint their physiological and pathological roles. This review presents an overview of EV, with focus on exosomes, the most studied EV, and on the cellular processes that mediate their biogenesis. Silicone is an adjuvant that may bleed and subsequently may be a chronic stimulus to the immune system. \u0000 \u0000Objective: To determine the prevalence of autoimmune diseases (AID) and risk factors associated to patients with ASIA induced by SBI (ASIA-SBI). \u0000 \u0000Patients and methods: This study was performed between 2012 and 2018, in a tertiary referral center, from a cohort of 210 patients with ASIA. We selected those patients with ASIA-SBI and clinical manifestations of any ARD. We investigated family history of AID, smoking, allergies and comorbidities. Statistical analysis: Chi square and Odds Ratio (OR). \u0000 \u0000Results: There were 45 women with current age 50 (29-75) years, mean time of appearance of clinical manifestations after SBI was 8.8 ± 5.5 years. We found systemic sclerosis (SSc) 10 patients, rheumatoid arthritis (RA) 8, undifferentiated connective tissue syndrome (UCTDS) 6, fibromyalgia (FM) 5, systemic lupus erythematosus (SLE) 4, Sjogren syndrome (SS) 3, angioedema/urticaria 3, overlap syndrome 2, and one of each of the following: Takayasu arteritis, Still disease, tunnel carpal syndrome and antiphospholipid syndrome. We observed family history of ARD 42%, allergy history 37.5% and smoking 35.5%. In ASIA-SBI patients, family history and smoking had a significant association with SSc OR 6.0 (CI 1.2-29), p<0.02 and RA OR 7.1 (CI 1.3-37.2), p<0.022. \u0000 \u0000Conclusions: In patients with ASIA-SBI, family history of AID and smoking were associated with SSc and RA. Data were collected every 3 months, including sociodemographic characteristics, functional status, disease activity and treatment. The primary outcome was the time to remission with the combined therapy, defined according to disease activity index of 28 joints (DAS28). Combination treatment failure was defined as not achieving remission. Time of outcome was assessed from date of the start of MTX plus LEF combination to date of remission or last follow-up. The Kaplan-Meier product limit method was used to estimate the probability of each outcome. \u0000 \u0000Results: A total of 106 patients were included. The median disease duration was 4 (IQR 2-8) months. Median followup was 34 ± 18 months (300 patients-year). Mean age was 50 ± 12 years and 83% were female. At the time of LEF addition, 95 (90%) of patients were not in remission. During follow-up, 47 (50%) achieved clinical remission at a median time of 8 months. The rest of the patients failed to achieve remission. The overall probability of remission per patients-months of follow-up was 0.49. Twenty seven out of 47 patients (58%) were still in remission at the last follow up visit (median follow-up after first remission=19 months). \u0000 \u0000Conclusion: In early RA patients with non-response to MTX monotherapy, the addition of LEF allowed to achieve remission in 50% of patients, at a median time of 8 months. In fact, T2T can be regarded a common place as to set and pursue goals since ever is of course essential in any aspect of curative medicine; nobody expects physicians to behave like blind persons in a dark forest. If T2T, although, is understood in the way that the treatment goal to achieve is a certain number on a scale, as it may be required by some originators, you may better forget about, as your patients will only have little understanding for this approach. Apparently the more the physicians lack clinical expertise they think to be in the need of such cloistered proposals. Besides knee is the most commonly involved peripheral joint in osteoarthritis [OA]. The aim of study was to investigate the concordance of patellar tendinopathy in patients with primary knee OA. \u0000Material and methods: Seventy eight [7 men and 71 women] patients with knee OA aged between 35-75 were included in the study. Demographic, clinical data, bilateral tenderness of the PT, visual analog scale [VAS] for pain assessment and ultrasonographic findings were recorded. The patients were divided into three groups according to \u0000grade of knee OA [grade 2, 3 and 4]. All patients’ PT examined with ultrasonography [US] by the same investigator. \u0000Results: The grade 2, 3, 4 OA groups were included 20, 42, 16 patients respectively. Significant correlations were detected between the proximal thicknesses of both knees (average) and age (p=0.000, r=0.395), weight (p=0.001, r=0.382), BMI (p=0.021, r=0.261) and VAS (p=0.002, r=0.344). There were statistically significant difference and \u0000positive correlation between measurements of PT thickness and grade of OA comparing with grade 2-3 OA (p=0.001). Furthermore there were statistically significant correlation between the tenderness of PT and proximal and distal PT thickness measurements in same knee (r=0.244, p=0.031; r=0.416, p<0.001, respectively). We did not detect any \u0000Power Doppler [PD] signal in 78 patients’ PT. \u0000Conclusion: The age, weight, BMI and severity of OA were independent risk factors of patellar tendinopathy. Also US is reliable imaging technique for detect patellar tendinopathy in patients with knee OA. Clinically, three variants have been described: cheilitis glandularis simplex, cheilitis glandularis suppurativa, and cheilitis glandularis apostematosa. Most cases are diagnosed as the simple form of glandular cheilitis, which is characterized by enlarged excretory ducts, induration, and enlargement of the salivary glands as well as production of a rather mucopurulent saliva. The diagnosis of CG is done with clinical and pathologic correlation. A swollen lip is common in clinical practice, but the differential diagnosis and management of the condition may be challenging. Lip swelling may be related to a local or systemic condition. We hereby report an unusual case of CG of lips in a female patient visited the deparment of Internal Medicine, Ege Univercity. She was being followed up with the diagnosis of rheumatoid arthritis and presented with recurrent swelling of the lips repeatedly. She had previously been followed up for angioedema because of recurrent swelling of the lips. The patient's laboratory tests were normal and lip biopsy was consistent with glandular cheilitis.
