Some leaks presents in a dramatic fashion early in the postoperative period, leaving little doubt about the diagnosis. However, many others present in a far more subtle fashion, often relatively late in the postoperative period, and can be difficult to distinguish from other postoperative infectious complications. We report a case of a 66 year old male who presented with bleeding per rectum since 3-4 months. He had a history of being operated for hemorrhoidectomy 9 years ago. He underwent a colonoscopy which detected a large sessile polyp at the rectosigmoid junction for which polypectomy was done. The histopathology of the polyp revealed moderately differentiated adenocarcinoma arising within the villoglandular polyp (malignant polyp) with base involved. He was then operated for anterior resection followed by colorectal anastomosis. Until postoperative day 5, the patient had no symptoms and signs of having an anastomotic leak, after which he complained of extreme abdominal discomfort, sudden distension and the drain had collected 35ml of feculent matter. In this way, adequate nutritional support helps in the healing stages, as well as in the patient’s quality of life. Objective: To report a case that presented an experience of using a specific supplement for healing as part of the treatment of surgical wound healing. A hyperproteic supplement was used, rich in arginine and proline, two amino acids that help in tissue recovery, in addition to containing specific vitamins and minerals, indicated for wound healing, together with a special dressing. Results: After 48 days of treatment, the patient developed granulation tissue in the bloody areas (100% granulation tissue). A reduction in depth was observed due to tissue regeneration, an increase in granulation tissue and epithelialization, in addition to a decrease in exudate. And in 82 days, complete healing of the wound. Here, we assessed the levels of the serum laboratory biomarkers in the patients with complicated (defined by the presence of perforation, abscess, or peritonitis) and simple appendicitis. Methodology: This observational study was conducted in Sina Hospital, Tabriz-Iran, between March 2019 and August 2019. Data regarding age, sex, body temperature, length of hospital stays, clinical signs and symptoms, and time of symptom onset for each patient. Laboratory values including white blood cells (WBC) count, neutrophil percentage, erythrocyte sedimentation rate (ESR), C-reactive protein (CRO), mean platelet volume (MPV), and lactate dehydrogenase (LDH) were evaluated. Results: A total of 279 patients were enrolled in the study. 131 patients (46.9%) had complicated and 148 (53.0%) had simple appendicitis. Mean body temperature (P-value <0.05) and CRP levels (P-value <0.0001), were significantly higher in the complicated appendicitis group. (P-value >0.05). There were no significant differences regarding WBC, neutrophil, 1st and 2nd hour ESR, MPV, and LDH levels, and mean length of hospital stay (P-value>0.05). Here, we performed a systematic review and meta-analysis to compare the efficacy and safety between thoracoscopic surgery and traditional open surgical approach for CDH. Methods: A systematic search of the electronic databases was conducted to identify studies compared the Thoracoscopic Repair (TR) and Open Repair (OR) for CDH. Parameters such as operative time, post-operative mortality, incidence of hernia recurrence, rate of patch use and post-operative complications were pooled and compared by meta-analysis. Results: Among the 712 children with CDH included in the twelve studies, 309 had received TR and 403 OR. All studies were non-randomized controlled trials. There were shorter operative times with the OR compared with TR [95% Confidence Interval (CI) 15.83 to 66.75]. The TR group had a significantly lower rate of post-operative death [95% Confidence Interval (CI) 0.09 to 0.63] but a greater incidence of recurrence [95% Confidence Interval (CI) 1.88 to 5.71]. Rates of prosthetic patch use were similar between the two groups. Fewer cases of surgical complications were found in the TR group [95% Confidence Interval (CI) 0.21 to 0.67]. Conclusion: Thoracoscopic repair for CDH is associated with lower post- operative mortality and longer operative times compared with traditional open repair. Although the rate of surgical complications appears to be lower in TR, the increased risk of CDH recurrence should not be ignored.
