CUTANEOUS ROSAI-DORFMAN DISEASE – A CLINICAL CASE 皮肤蔷薇-多夫曼病1例临床分析

Rosai-Dorfman disease is a benign condition, characterized by self limiting proliferation of non-Langerhans histiocytes insidelymph nodes but also in extranodal areas. Clinical case. A 54-year-old patient, diagnosed with arterial hypertension since 2011, is hospitalised in the Dermatology Department of the Emergency County Hospital, Craiova, Romania, in March 2016, for a polymorphous rash composed of violet-erythematous papulonodules with sizes ranging between 0.2 cm and 2 cm, slightly itchy, located in the latero-thoracic region, lumbar region and in the area around the menton area. The lesions appeared 18 months prior to hospitalization. Clinical and paraclinical investigations as well as interdisciplinary consultations revealed no associated pathologies. Histopathological and immunohistochemical examinations confirmed the diagnostic of cutaneous Rosai-Dorfman disease, imaging investigations excluding other localizations. Treatment with Doxycycline, in combination with 20 mg of Rutoside and 50 mg of ascorbic acid, Pentoxifylline and Levocetirizine resulted in remission of the skin lesions. Discussion. Rosai-Dorfman disease is a rare condition, having a reported incidence rate of 1:200.000. It generally affects males, being more frequent in young males and people of African descent. The pathogenesis of Rosai-Dorfman disease is still not fully understood, although immunological and infectious factors may play an important role. Foucar has divided this condition into three types: nodal, extranodal and mixed. Cutaneous lesions have been described in 3% of cases of Rosai-Dorfman disease of the extranodal type. It is more frequent in females (sex ratio of 2/1) and in adults of Asian descent, with a mean age at onset of 45 years. Treatment options include: oral and intralesional corticosteroids, alfa-interferon, retinoids, dapsone, methotrexate, thalidomide, cryo- and radiotherapy. Conclusions. The presented case is an atypical one because of the three locations of the skin lesions. Although we obtained remission of the disease, the patient would remain in direct observation because of the risk of recurrence. Glomus tumors are rare and mostly benign lesions of the subcutaneous tissue. Single lesions are preferably located on the acral parts of the body, including the subungual nail apparatus. Observations. We analyzed our files for glomus tumors of the last 10 years and found four tumors, with an equal gender distribution. The patients’ age ranged from 41 to 78 years. The dominant site was subungual on fingers. Advanced and long-standing tumors tend to destroy the overlying nail plate. Another tumor was found on the plantar heel. All patients had a delayed diagnosis between 3⁄4 of a year to three years. Nail plate dermoscopy is a useful tool for diagnosis of early subungual lesions. All tumors were successfully treated surgically with rapid and complete pain relief. Histologically, all four tumors were of the glomangioma subtype with predominant vascular structure. No relapse occurred within a follow-up of up to nine years. Conclusions. Glomus tumors are rare. In case of subungual localization, nail plate dermoscopy is a useful non-invasive diagnostic tool. The treatment of choice is complete tumor removal by surgery. A better knowledge of this tumor will help to reduce the time to final diagnosis and treatment. Herpes zoster clinically presents as a unilateral, painful, vesicular eruption, usually distributed to one or two adjacent thoracic dermatomes. As it is rarely seen confined to the upper limbs, few reports of herpes zoster involving the ulnar nerve exist in the literature, the majority describing patients who present with segmental zoster paresis of the upper extremity. This paper reports the case of a 73-year-old woman with ulnar and posterior antebrachial cutaneous nerve zoster reactivation and no history of immunosuppressive disorders. Therefore, rare occurrences of herpes zoster, such as the involvement of cervical dermatomes, should also be considered when faced with the differential diagnosis of symptomatic skin rashes of the upper limb, especially in elderly or immunosuppressed patients. To demonstrate that vaginal tightening results improve at the progestational phase of the cycle. 2. To assess if menstrual cycle can be a conditioning factor in the results of vaginal tightening. Method: Descriptive, comparative, cross-sectional study conducted on 20 women who attended consultations between October 2017 and December 2017. Two sessions with fractionated CO 2  laser and intravaginal handpiece were performed. The day of the cycle was taken into account to carry out the vaginal tightening, and Likert’s scale was applied to the month of each session. Results: The sample group included 20 women, most of them between 29 and 39 years of age (60% = 12 cases). A statistically significant association between the phase of the menstrual cycle and the level of friction during sexual intercourse after the second session was found. Women who reported a high degree of friction during sexual intercourse at the second tightening session registered a higher average of days of menstruation than those who reported a moderate degree of friction. This difference was statistically significant. Conclusion: It was possible to determine in the study that the progestational phase improves the results of vaginal tightening with up to 100% and that menstrual cycle can be considered a conditioning factor for the results.
