Endoscopic resections can become challenging or impossible even if the tumor was located near esophagogastric junction. Performing gastrectomy for these mostly benign lesions will also be a rather brutal treatment method in most cases.Therefore, alternative minimal invasive resection techniques and their reliability should be evaluated. The aim of this study is to evaluate the efficacy and safety of endoscopy-assisted laparoscopic transgastric resection method in proximally located submucosal tumors. Materials and methods: Transgastric combined endoscopic and laparoscopic surgery (CELS) with intragastric port was performed in one patient and Transgastric CELS with gastrotomy was performed in six patients who had tumor located near esophagogastric junction at XXX Hospital between February 2019 and February 2020. Results: The patients were included 3 men and 4 women with and average age of 45.8 years (range 25-70). In five of the cases, four ports and nathanson retractor were used for liver retraction. While three ports were used in one patient, the stomach was suspended with traction suture. In one patient, 5 ports were used. The average operation time was 88 minutes (range 59-140 min). While gastrostomy line was closed with linear stapler in two patients, laparoscopic hand sewing method was used in the remaining patients. No patient had intraoperative complications. Conclusion: We strongly suggest that the laparoscopic transgastric resection approach for submucosal tumors close to the gastroesophageal junction, is a feasible and safe method, when used in combination with endoscopy.","PeriodicalId":321125,"journal":{"name":"Medical Journal of Bakırkoy","volume":"128 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121389952","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Mode of delivery has a marked effect on weight loss in the immediate postpartum period. The aim in the present study was to construct weight loss percentile charts according to mode of delivery in exclusively breastfed, healthy term infants during hospital stay. Method: Weight loss in a large Turkish cohort of infants, born between January 1, 2011 and December 31, 2014, was evaluated retrospectively. Data on healthy, term and exclusively breastfed neonates during the immediate postpartum hospital stay were collected. Weight change percentile charts were plotted according to mode of delivery. Results: The study encompassed 3247 exclusively breastfed neonates. Of infants 48.1% were girls. Mean gestational age was 38.94±0.84 (range 37-41) weeks and birth weight of infants was 3381.1±380.9 (range 2150-5190) grams. The rate of caesarean delivery was 69.3%. The time of hospital stay of infants born by caesarean delivery was significantly longer than infants born vaginally. The frequency of weight measurements of infants showed a statistically significant difference according to the type of delivery. Weight loss as a percentage of birthweight for infants born by caesarean delivery were significantly greater at 24, 48, 72 and 84 hours after birth compared to those born via vaginal delivery. Conclusion: Plotted percentile charts according to mode of delivery will enable prediction of early weight loss immediately post-partum. In addition, these percentile charts will help to reassure mothers and encourage breastfeeding exclusivity.","PeriodicalId":321125,"journal":{"name":"Medical Journal of Bakırkoy","volume":"5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128407711","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Method: Demographic features, underlying etiologies, treatment modalities, electroencephalographic and neuroimaging outcome of intensive care unit admissions between 2015 and 2019 were analyzed. Results: Seventy-one children were enrolled. The common etiology for prolonged seizure was fever in 45.1% of the children (central nervous system infection: 16.9%, infection other than central nervous system: 28.2%), withdrawal of the antiepileptic medication in 40.9%, intoxications in 12.7% of the children and intracranial hemorrhage due to arteriovenous malformation in one (1.4%) patient. Initial diagnostics exhibited 23.9% of hypoglycemia, 18.3% of hypocalcemia, 15.5% of hyponatremia, 35.2% of acidosis, and 25.4% of elevated lactate. Group categorizations were based on fever and new onset of seizure. Approximately 45.1% of the population acquired fever at the onset and consisted of younger children compared to their counterparts (p=0.023). Children with pre-existing epilepsy had frequent history of sibling death, while patients with new-onset of seizure possessed significant lactate elevations, acidosis, and required mechanical ventilation more often (p=0.002, p=0.008, p=0.017). Twelve (16.9%) patients developed SRSE. Low serum sodium and calcium levels were associated with developing SRSE (OR: 10.800, 95%CI: 2.518-46.318; OR: 4.554, 95%CI: 1.159-17.892); however PRISM-3 score has been identified the single independent risk factor in children proceeding to SRSE (OR: 1.174, 95% CI: 1.039-1.327). Three (4.2%) children died of secondary complications. Tracheostomy cannulation was performed in two (16.7%) SRSE patients due to neurological incapability to maintain a patent airway. Conclusion: PRISM-3 score is the independent risk factor of SRSE. Electrolyte abnormalities (hyponatremia and hypocalcemia) are associated with developing SRSE.","PeriodicalId":321125,"journal":{"name":"Medical Journal of Bakırkoy","volume":"18 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125186256","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Method: A total of 104 confirmed COVID-19 patients aged between 0-18 years were retrospectively analyzed. The study period was between March 15, 2020 and July 1, 2020. A confirmed COVID-19 was defined as a child who has positive real-time reverse transcription-polymerase chain reaction (rRT-PCR). The patients were analyzed in terms of demographic, epidemiological, clinical, laboratory and imaging features, and clinical outcomes. Results: The median age (inter-quartile range) of the patients was 11.8 (8.4) years, and 53 (51%) were male. Ninety-eight (94%) of the patients had an epidemiological risk factor. The most frequent symptoms were cough (62%), fever (42%), and fatigue (33%). Seven patients (7%) were asymptomatic, 84 (81%) were mild, 9 (9%) were moderate, and 4 (4%) were in severe or critical condition. Ninety-eight (94%) of the patients underwent a chest x-ray, and 12% had pneumonia. The patients younger than 5 years were more likely to have fever (p = 0.019), rhinorrhea (p = 0.041), and diarrhea (p = 0.035). Muscle/join pain (26%), loss of smell/taste (20%), and headache (19%) were striking extra-pulmonary symptoms in older patients. Only 8% of the patients were hospitalized, and 4% were admitted to the intensive care unit. No death occurred. Conclusion: The most of the pediatric COVID-19 cases have mild symptoms without pneumonia. They may not present with fever or cough which are considered characteristic symptoms of the disease and the frequency of signs and symptoms may vary by age. Children younger than 5 years old are more likely to have fever, rhinorrhea, and diarrhea. School-age children and adolescents may present with extra-pulmonary symptoms such as muscle/join pain, smell/taste loss, and headache.","PeriodicalId":321125,"journal":{"name":"Medical Journal of Bakırkoy","volume":"62 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134563674","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In this study, we aimed to evaluate the factors associated with 28-day outcomes in patients admitted to the intensive care unit with the diagnosis of COVID-19. Methods: This study has a retrospective cohort design. COVID-19 patients identified according to World Health Organization guidelines are included. Patient data were recorded to a centralized system utilizing ImdSoft-Meta vision/QlinICU Clinical Decision Support Software. Individual datasets about required parameters were obtained from Structured Query Language (SQL) queries. The main laboratory parameters were examined. SOFA, APACHE II, and Charlson Comorbidity Score (CCS) were calculated. In evaluating laboratory parameters and disease risk scores, which are thought to affect 28-day mortality, logistic analysis were performed using the Backward LR model. Results: The study was carried out with 101 patients, 40 (39.6%) of whom were women, and 61 (60.4%) of men, who met the inclusion criteria. The ages of the patients ranged from 21 to 88, and the mean age was 58.45 ± 15.41 years. The mean intensive care hospitalization period was 12.5 ± 10.2 days. The all-cause in-hospital mortality rate was 61.4%. Leukocyte count, CK, NT-proBNP, PCT, CRP, ferritin, neutrophil count and percentage, D-Dimer, LDH, AST values were found to be significantly higher in non-survivors. The lymphocyte count and percentage, and platelet count values were found to be significantly low in non-survivors. The lymphocyte percentage, LDH, and CCS were significant in the 28-day mortality in multivariate analysis (p values are 0.01, 0.003, 0.008, respectively). Conclusions: High lymphocyte values have been found to significantly reduce the risk of death in patients diagnosed with COVID-19. Lymphocyte percentage, LDH, and CCS were evaluated as the most successful parameters in predicting 28-day mortality in the intensive care unit.","PeriodicalId":321125,"journal":{"name":"Medical Journal of Bakırkoy","volume":"66 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134233054","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We aimed to examine the results of prostate artery embolization (PAE) treatment for BPH in the elderly patients with high comorbidity. Materials and Methods: In the present study, we evaluated the patients with lower urinary tract symptoms (LUTS) due to BPH and applied endovascular embolization treatment, who were admitted to our hospital radiology department, between February 2017 and February 2019. Patients with a high risk for surgical operation with an American Society of Anesthesiologists (ASA) score of 3 or above, International Prostate Symptom Score (IPSS) >20 and MR (Magnetic Resonance) prostate volume (PV)>70 cm3 were included in this study. In addition, maximum flow rate Qmaximum (Qmax), MRI PV and IPSS values were recorded in all patients in our clinic before PAE, and the 12th months after PAE. Results: In our study, 15 patients, who had a history of transurethral resection (TUR), and 57-82 years old (mean 73.8), participated. PAE treatment was administered to the patients who were admitted to our hospital with the diagnosis of BPH in patient high comorbidity, LUTS and high risk for open prostatectomy. In our series, Qmax, IPSS, MRI and digital subtraction angiography (DSA) findings were presented. The findings showed that pre-PAE and post-PAE at 12th month, IPSS value 25,13±1,92 and 11,4±6,51 (p=0,001), Qmax; 7,61±3,19 and 16,51±9,56 (p=0,001), MRPV; 135,47±76,48 and 83,27±43,53 (p=0,001) and also all parameters were statistically significant.
