Panesar","doi":"10.5772/intechopen.92805","DOIUrl":"https://doi.org/10.5772/intechopen.92805","url":null,"abstract":"Trauma and orthopedics is a specialty in which significant blood loss can be incurred both in terms of traumatic injuries and operative management. This chapter starts with a brief review of the biology of hemostasis followed by the importance of hemostasis in surgery. This is followed by a discussion on the ideal hemostatic agent. Various strategies of achieving hemostasis will be discussed including mechanical, thermal, pharmacological and topical agents in both elective orthopedic and spine surgery as well as in trauma. Specifically, we will look at synthetic agents such as cyanoacrylate, polyethylene glycol hydrogel and glutaraldehyde cross-linked albumin and absorbable agents such as gelatin foams and oxidized cellulose. We will also look at biological agents such as topical thrombin, sealants and platelet gels. Within the realm of cardiothoracic surgery, quality is synonymous with technically excellent, safely conducted operative procedure followed by an uneventful patient recovery and follow-up. Critical to this process of clinical value creation is meticulous attention to all aspects of every step along the management continuum. From surgical quality improvement perspective, the reduction in blood loss and thus minimization of blood/blood product transfusions are of critical importance. Biosurgicals have recently drawn attention to both the control of bleeding and chronic wound management. However, their role will be examined here in the context of adjuncts to control preoperative, intraoperative, and postoperative bleeding in trauma. A review of the scientific literature relevant to the use of passive and active topical hemostatic devices, as well as systemic pharmacologic agents, for control of hemorrhage is provided, in both military and civilian contexts. Bibliometric publication patterns and published guidelines are examined to identify the range of individual products available and the degree of attention they receive in the management of acute traumatic injuries. Blood and its products are the most valuable at the same time dangerous arms of surgeons. Industries produced many biological or non-biological products, which increase the cost of operations, are available for this issue. Decision making and producing solutions must be the most important properties of a cardiac surgeon. In any condition and resources, true strategy and patience will bring the success. Antiaggregants and anticoagulants are widely used agents, and in emergency or early cases, surgeons have to operate these patients. Both groups of drugs are growing, and in an emergency position, it is hard to know which drugs the patient using or given and what kind of medical status he/she has. For example non vitamin-K oral anticoagulants newer drugs and not known so much except cardiologists and cardiovascular surgeons. Simple solutions may have better results. Of course, in some conditions, urgent consultation can solve the problem. Cardiac surgeons have to figure out the problem by using their experience and surgical materials.
