Children on the autism spectrum (AS) face unique challenges that arise from their difficulty in understanding social behavior and interaction, difficulty in understanding and effectively using communication, and difficulty with having reduced flexibility of thought and behavior. A large number of studies have reported that parents of children on the AS experience higher levels of stress when compared to parents of typically developing children and children with other developmental disabilities. Such stress levels are dependent on a number of factors which seem to be interrelated and complex. {"title":"Severe and enduring Anorexia Nervosa: a brief narrative review about the concept and its therapeutic options","authors":"F. Coutinho, I. Brandão","doi":"10.21035/IJCNMH.2019.6.3","DOIUrl":"https://doi.org/10.21035/IJCNMH.2019.6.3","url":null,"abstract":"Anorexia Nervosa (AN) is a severe psychiatric disorder that can present in a chronic and durable form, often named severe and enduring AN (SE-AN), with little available literature considering therapeutic options in this subgroup of patients. We aimed to perform a bibliographic review considering articles about severe or chronic forms of AN published in the last ten years. According to the analyzed data, there are different definitions in the studies concerning SE-AN, however, many authors consider 7-years of disease as a threshold for diagnosing SE-AN. In the future, it will be important to have a standardized definition of SE-AN in order to characterize its preferential approach in large scale studies. Regarding therapeutic options, cognitive-behavior therapy remains a valuable option in these patients, despite the need of some adaptations considering the high probability of past failed psychotherapeutic approaches. Regarding biological therapies, there is now exploration of new possibilities such as the use of transcranial magnetic stimulation and deep brain stimulation in these patients. Its prevalence is around 2 to 3% in the general population, and despite being responsive to treatment, 40-60% of patients are not full responders and remain refractory. Therefore, new therapeutic options like transcranial magnetic stimulation (TMS) have been gaining interest recently due to their non-invasive nature and potentially promising results. However, there is not yet a consensus about its efficacy in this disease. Parents are usually the primary caregivers for their children and the essential link between home and school environments. Often they also take on a coach or therapist role across a variety of interventions, which demands a great amount of time, dedication, energy and financial resources. Increased parent skills allow for continued opportunities for the child to learn in a range of different situations and environments, and parent training is now considered an important component of successful intervention programs.
