An investigation into the neural mechanisms involved revealed that the subsequent increase in pulmonary blood flow stimulates J or juxtapulmonary capillary receptors that lie in the pulmonary parenchyma. Reflexes arising from stimulation of these sensory receptors are an increase in ventilation accompanied by sensations of breathlessness, bradycardia and reduction or stopping of exercise. Of these, the significance of reflex bradycardia is to reduce the great rise in pulmonary capillary pressure during strenuous exercise and protect the pulmonary arterioles from rupture as they are vulnerable to mechanical stress.","PeriodicalId":300618,"journal":{"name":"Mathews Journal of Cardiology","volume":"19 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123492937","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, its evolution is very unfavorable when it occurs. The appearance of a fever, often insidious, is a sure sign of the disease. Diagnosis is based on blood cultures and echocardiography. This clinical case is an opportunity for us to recall the seriousness of this condition both for patients and for cardiac surgeons. Methods: We report the case of an adult aged 51 with two mechanical mitro-aortic prostheses implanted in 2001 presenting endocarditis on aortic prosthesis complicated by aortic leak, atrioventricular block (AVB) and stroke leaving as a sequela a hemiplegia with infectious aneurysm of the right sinus ruptured in the right atrium (RA) in cardiac decompensation with persistent fever and orthopnea despite well-conducted triple anti-staphylococcal antibiotic therapy with clinical examination: aortic systolic murmur with crackling rales. Chest X-ray: cardiomegaly, flaky opacities, transthoracic echocardiography: large aneurysm on the right coronary side fistulized in the right atrium (RA), desinsertion of the aortic prosthesis with grade IV paraprosthetic aortic leak, left ventricular (LV): 52/32 mm, an undilated right ventricular (RV), an ejection fraction (EF) at 64 %, and finally a systolic arterial pulmonary pressure (SAPP) at 68 mmHg. Positive blood cultures: staphylococcus. Intraoperative exploration: voluminous vegetation next to the exit orifice of the aorta-right atrium fistula, desinsertion of the aortic prosthesis on the peri-annular abscess and vegetation on the aortic wings, destruction of the mitro-aortic junction with the presence of a fistula aorta-right atrium. He benefited from explantation of the aortic prosthesis, vegetation sent to bacteriology, reconstitution of the aortic annulus on the right coronary-left coronary (RC-LC) side by a Dacron® patch, closure of the entry orifice of the fistula on the aortic side by separate points and reconstruction of the mitro-aortic junction with a triangular Dacron® patch, implantation of an aortic prosthesis *Corresponding Author Late Endocarditis on Aortic Prosthesis Complicated by an Infectious Aneurysm of the Right Sinus Ruptured in the Right Atrium: A Case Report","PeriodicalId":300618,"journal":{"name":"Mathews Journal of Cardiology","volume":"40 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115307701","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Conventional treatment methods for restoring sinus rhythm include synchronous electrical cardioversion, pharmacological cardioversion and radiofrequency ablation, which are effective but still limited due to treatment failures to control sinus rhythm and the side effects of long-term medication and the invasiveness of radiofrequency ablation. Ventricular tachycardia (VT) is a malignant arrhythmia, which is the most common cause of sudden cardiac death (SCD). Antiarrhythmic medications and endocardial catheter ablation remain as guidelines-supported VT treatment strategies for patients with structural cardiac abnormalities. Recent clinical results showed that more than half of patients had recurrent VT after catheter ablation, and for patients with intracardiac thrombosis, deep ablation sites, or mechanical valve replacement, catheter ablation is often not completed. The invasive and potential complications of radiofrequency ablation make it unattractive for patients with multiple complications or hemodynamic impairments. body radiotherapy technique and its clinical potential for the noninvasive ablative treatment of AF and VT together with its important implications for future beneficial clinical applications. In vivo experimental studies that provided useful preclinical data are reviewed and the results of pioneering clinical trials will be discussed together with the dosimetric considerations and pros and cons of the treatment approach.","PeriodicalId":300618,"journal":{"name":"Mathews Journal of Cardiology","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126535740","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Most tumors are solitary, and multiple intracardiac myxomas account for less than 5% of all cases of myxoma. Moreover, valvular myxomas are exceedingly rarer. Therefore, we report unusual myxomas with an unique anatomical location, which emphasizes the value of multiple imaging investigations in the diagnostic workups and subsequent management.","PeriodicalId":300618,"journal":{"name":"Mathews Journal of Cardiology","volume":"7 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130644000","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is defined as a transient constriction of the muscles in the wall of one or more of the coronary arteries. Myocardial injury and sudden cardiac death are possible remarkable sequels. Despite coronary spasm is a brief and temporary but can be permanent. In other words, it may be associated with or without significant coronary artery disease. However, Coronary spasm is hyperresponsive to the vasodilator effect of nitrates. Patients with different forms of coronary artery disease may respond differently. The quick reversibility response of ST-segment depression after using short-acting nitrates had achieved in my 3-case series. ST-segment depression reversibility with nitrates means that the ST-segment depressions for all cases were transient coronary vasospasm. However, coronary angiography was normal in all 3-cases. This is the principal of our test. Worthwhile, the objective of the test in the current article is to distinguish between the real coronary artery spasm with obstructive coronary artery disease from those with nonobstruction. The emergency differentiation for the cases of coronary artery spasm using nitrates via categorical stratification into obstructive and nonobstructive coronary artery disease is a pivotal step in future management. The test may reduce the need for coronary angiography and its economic impact on the patient.","PeriodicalId":300618,"journal":{"name":"Mathews Journal of Cardiology","volume":"13 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133333319","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
