Treatment of Pemphigus Vulgaris and Pemphigus Foliaceus: Experience with 30 Patients 寻常型天疱疮和叶状天疱疮30例的治疗体会

This study focuses on the age, gender, mucosal lesions, therapy with oral prednisone, clinical remission rates and side effects. Material And Methods : Six males/15 females with Pemphigus vulgaris and three males/six females with non-endemic pemphigus foliaceus attended at Hospital Universitario from 2003 to 2018 were submitted to histopathology through skin or mucosa biopsy. Oral prednisone (monotherapy) in seven Pemphigus vulgaris and two pemphigus foliaceus; in case of no control or relapses, association with cyclophosphamide pulse therapy (12PV/4PF), dexamethasone pulse therapy (ten PV/two PF), hydroxychloroquine (three PV/four PF), azathioprine (five PV),methotrexate (four PV/one PF), immunoglobulin cycles (two PV/two PF). Results : Mostly patients were female aged 30-59 years old; oral mucosal lesions in 19 (90,4%) pemphigus vulgaris. After a ten year period of monitoring it was observed in the group of monotherapy: one PV/one PF achieved complete remission on low dose of prednisone; two PV achieved partial remission on low dose and four PV/one PF on high dose. In the group with combined drugs, four PV were off prednisone, four PV/one PF achieved complete remission on low dose and two PF on high dose; two PV/three PF achieved partial remission on low dose and four PV/one PF on high dose. The most common side effects of prednisone were mucocutaneous candidiasis, arterial hypertension, subcapsular posterior cataracts, bacterial and viral infections and diabetes mellitus. Conclusion : Both diseases ran a chronic and unpredictable course which management was extremely difficult. A 49 – year old male patient with pemphigus vulgaris developed in the third year of his disease a hypertrophic plaque in the scalp. Lesion ́s biopsy has revealed acanthosis, papillomatosis of the epidermis and suprabasal clefts; a diagnosis consistent with Pemphigus vegetans was made. The patient was then put on shaving, mechanical debridement, clobetasol propionate and sulfamethoxazole – trimethoprim after the isolation of Staphylococcus aureus in biopsy fragment of the scalp lesion. After increasing azathioprine dose and starting with papain dressings on the long – standing lesion it was observed a slow and progressive clearing. A chronic infectious process linked to Pemphigus vegetans might explain the refractoriness to multiple therapeutic approaches. It is an idiopathic disease, relatively rare, which has been described mainly in black people. It consists of a spontaneous dactylosis [2] that leads to the self-mutilation of one or more fingers, generally although not exclusively, of the fifth toes. Its etiology is unknown, although factors such as chronic trauma from walking barefoot has been associated with its physiopathogenesis [3]. The goals of this study were: to determine demographical and clinical characteristics of patients with eyelid eczema patch tested between 2004 and 2018; to determine established diagnoses; and to identify responsible allergens. Medical records of patients with eyelid eczema who underwent patch testing were analyzed. This study included 228 patients, of which 89.5% were women, with a mean age of 45 years. Regarding clinical condition, 64.5% presented eczema lesions in other body parts besides the eyelids, mainly in other facial sites (51.8%). Final diagnosis was allergic contact dermatitis (ACD) in 61%, atopic dermatitis (AD) in 12.7%, undefined in 12.3%, irritant contact dermatitis in 7.9%, superimposing of ACD and AD in 3.1%, and other diagnoses in 3.1% of patients. Regarding patch tests, 64.4% of patients had at least one relevant positive allergen, the majorones being: toluene-sulfonamide-formaldehyde resin, paraphenylenediamine, nickel sulfate, fragrance mix I, neomycin, and Kathon CG. As main etiologies in ACD cases, nail polish (36%), topical medicaments (27.2%), non-specified cosmetics (24.5%), hair dye (13.6%), metals (15.6%), rubber (6.8%), and shampoos (4%) could be identified. Thereby, results presented compatibility to data in the literature: predominance of women and most prevalent final diagnosis of cosmetic-related ACD. Although its location in the periorbital area is distinctive, its course can be systemic and generalized. In the beginning, the diagnosis is difficult and its clinical-pathological correlation becomes necessary. Its prognosis is generally good, but currently there is no standardized or effective treatment;surgery is an option yet with high tendency for recurrence. {"title":"Girdled and Hemmed In -Perivascular Epithelioid Cell Tumour","authors":"Anu Bajaj","doi":"10.47690/SWJSDV.2020.1103","DOIUrl":"https://doi.org/10.47690/SWJSDV.2020.1103","url":null,"abstract":"PREFACE Exceptional neoplasia with perivascular clear cell and epithelioid cell differentiation are diverse and constituted by angiomyolipoma, clear cell “sugar tumour” of pulmonary and extra pulmonary sites, clear cell myomelanocytic tumour offalciform ligament or ligamentum teres and lymphangioleiomyomatosis on account of distinctive histological manifestations. Variable genetic and malignant biological behaviour is exemplified within perivascular epithelioid cell tumours. Perivascular epithelioid cell tumour (PEComa) is a contemporary neoplasm of mesenchymal origin emerging within diverse sites while representing a classified group with distinctive histology and immune histochemical staining. The tumefaction is composed of epithelioid cells which demonstrate dual properties of smooth muscle and melanocytes with pertinent immune reactivity. Cellular component tends to aggregate around and circumscribe blood vessels, thus engendering the acronym “PEComa”. Perivascular epithelioid cell tumour was initially described by Bonetti et al in 1992 [1]. Zamboni et al in 1996 documented perivascular epithelioid cell tumour of the pancreas and denominated the neoplasm as “sugar tumour” on account of clear, glycogen rich, cytoplasm imbued within perivascular epithelioid cells.
