Therefore, it is not appropriate to use the presence of a Karhart’s notch as a pathognomonic sign of otosclerosis. Biletska","doi":"10.37219/2528-8253-2020-6-42","DOIUrl":"https://doi.org/10.37219/2528-8253-2020-6-42","url":null,"abstract":"The aim of this study is to assess the dynamics of immunoglobulin E and G under the influence of combined allergen-specific immunotherapy (ASIT) in patients with allergic rhinitis with combined sensitization to pollen and household allergens.\u0000Materials and methods: The study involved 49 patients, residents of Dnipro city, of working age (35,5±1,5 years), of whom 24 (49,0%) - women and 25 (51,0%) - men. All patients were diagnosed with allergic rhinitis with average disease duration of 9,2 ±1,1 year, who underwent allergy examination - skin tests - prick test with pollen and household allergens and/or methods of allergy component diagnosis ALEX. The number of pollen allergens in one patient was 4,6±0,4 on average. All patients received one course of pre-season ASIT allergen according to the express scheme. Patients in the main group (31 patients) received a combined ASIT with solutions of pollen and household allergens. 18 patients in the comparison group received ASIT only with pollen allergens. Clinical groups were homogeneous in age, sex, duration of the disease, as well as the average number of pollen allergens.\u0000Results: The obtained results and their analysis show that the use of allergen-specific combination immunotherapy in patients with allergic rhinitis with combined sensitization to pollen allergens causes a more significant effect on the activation of the immune response, which was revealed by reduced relative to baseline lgE production by 41,9% (p<0,001) and an increase in lgG by 7,5% (p<0,05). A rare case is described with contact mastoiditis combined with destruction of bone structures adjacent to the cranial cavity was secondary to multiple, huge, long-lasting, latent epidural abscesses. Popovich","doi":"10.37219/2528-8253-2020-6-49","DOIUrl":"https://doi.org/10.37219/2528-8253-2020-6-49","url":null,"abstract":"Topicality: The rate of acute bacterial rhinosinusitisis 0,5-2% of all cases of acute rhinosinusitis, however, up to 60 % of patients receive the course of antibiotics since the first day of disease. It is well known that earlier use of antibiotics has almost no effect on the development of bacterial ARS and its complications.\u0000The purpose of study was to examine the bacterial spectrum of post-viral RS and evaluate clinical and bac-teriological efficiency of polyvalent bacteriophage in technique of delayed prescribing of antibiotics in comparison with the patients receiving standard APVRS therapy.\u0000Material and methods: 155 adult patients with acute post-viral RS participated in the trial who were given either standard therapy or polyvalent bacteriophage in addition to the standard therapy. In conjunction with the treatment, the microbiological composition of nasal microflora was examined and its comparison with clinical dynamics was undertaken. Evaluation criteria: decrease in intensity of disease symptoms evaluated in accordance with MSS scale system during each visit in comparison with the 1st visit, antibiotic prescribing frequency, dynamics of microbiological changes.\u0000Outcomes and discussion: The use of polyvalent bacteriophagein addition to the standard therapy of acute post-viral rhinosinusitis as a part of technique of delayed prescribing of antibioticsensures clinically significant decrease in the intensity of rhinorrhea symptoms, nasal congestion and post-nasal drip on the third day of supervision (p<0.05). It reduces the prescription of antibacterial medicines by 20%without negative impact on overall treatment results. Decrease in the number of prescription of antibacterial therapy correlates with the fair reduction in bacterial loadwith Staphylococcus aureus and typical representative of ABRS – S. pneumoniae, Haemophilus influenza and M. catarrhalis. Therefore, the intervention group in comparison with the control group has had “therapeutic advantage” during the first days of treatment which allowed to assess the course of disease as a “positive” and avoid in appropriate use of antibiotics. There have been documented no patients with adverse effects during the course of treatment.\u0000Conclusion: polyvalent bacteriophage is a safe and effective medication for additional treatment of acute post-viral rhinosinusitis intended to reduce the use of antibiotics. Due to the significant increase in cardiovascular disease, endovascular interventions, as well as the wide use of anticoagulant and disaggregating therapy (in most cases, long courses or even lifelong), cases of recurrent NH have become more frequent. In reference, there is a problem of modification of local methods of stopping NH in patients who receive the above therapy for a long time, because intervention in the hemostasis system in this group of patients causes certain difficulties.\u0000Aim of the study: The development of an anatomical intranasal tampon that allows you to combine several types of effects on the bleeding area is an extremely important task.\u0000Materials and methods: We have proposed a combined method of stopping NH, which includes the use of a two-chambered hydraulic tampon of our self design in combination with gel “Nosochem” and thermal exposure.\u0000Results: The proposed method of stopping NH was used in 24 patients, the average duration of nasal tamponade was 52.4±5.3 hours. In 23 (95.8%) patients, the proposed method provided effective stoppage of NH.\u0000Conclusions. Kashirin","doi":"10.37219/2528-8253-2020-6-20","DOIUrl":"https://doi.org/10.37219/2528-8253-2020-6-20","url":null,"abstract":"Thiol groups of the intra and extracellular molecules are antioxidant and protect cells from damage caused by free radicals, but dynamics data of serum total thiol (sTT) levels in patients at laryngeal cancer are almost absent.\u0000Purpose of the study: Determination and comparison the serum total thiol levels at different stages of special treatment methods in laryngeal cancer patients.\u0000Material and methods: In twenty-eight laryngeal cancer (T2-3 NX M0 category) patients’ examination of the thiol status dynamics was carried out before, during and after the combined treatment with “ELISA” diagnostics test system.\u0000Results: Almost a twofold decrease in sTT content indicators was noted but no identified statistically significant differences between T2 and T3 categories tumors. There was an increase in sTT levels in patients in the postoperative and during radiation therapy period, which, however, were below the control values. At the same time, when the combined treatment was completed, a significant decrease in the level of sTT was noted, which emphasizes the need for long-term antioxidant therapy. Higher sTT levels have been associated with a reduced risk of laryngeal cancer recurrence.\u0000Conclusions: In laryngeal cancer patients, the level of serum total thiol was lower than in patients of the control group. Swelling of the nasal mucosa and paranasal sinuses accompanies sinusitis; but this is not only a symptom, edema leads to blockage of the normal sinus drainage pathway, which is key in the pathogenesis of sinusitis.\u0000Decongestants are considered a key treatment for sinusitis because they improve the function of the sinuses. According to EPOS-2012, treatment of acute sinusitis in children up to 5 days includes nasal lavage, decongestants, and in the absence of effect within 10 days, endonasal corticosteroids are added to the treatment.\u0000Decongestants are prescribed from the onset of sinusitis in children.
