In this study, we aimed to investigate the impact of a nine-day holiday on ED admissions and admissions due to cardiac complaints in our country, Turkey.\u0000Methods: This was a retrospective study. Patients were analyzed in 4 periods: Pre-Eid-El-Fitr, Eid-El-Fitr, pre-Eid al-Adha, and Eid al-Adha. Waiting times in the ED, the left without being seen (LWBS), consultations, admission diagnoses of cardiac patients, referral status of cardiac patients, and hospitalization were examined.\u0000Results: The most frequent visits to the ED were on Eid al-Adha (n=6581, 40.1 %). The waiting time before the ED examination increased with the crowd, and the longest waiting time was observed during Eid al-Adha (38.09 ±51.74 min, p<0.001). LWBS rate was the highest during the Eid al-Adha (15.4%). It was observed that cardiac emergencies had shorter waiting times. Among the cardiac emergency patients, all NSTEMI patients could not be referred from the ED during the Eid al-Adha period due to the fullness of the angiography centers, and the patients were transferred to the intensive care unit when the beds became available.\u0000Conclusion: Due to the crowded EDs during extended holiday periods, the triage system gains importance for the early detection and treatment of cardiac patients. Immediate reperfusion with hemodynamic support should be the primary goal in patients who develop impaired blood flow distal to the acute LMCA and the first option is coronary artery by-pass grafting. We present a 37-year-old female patient in the postpartum period who was admitted to the emergency department because of chest pain and was urgently admitted to our catheterization laboratory with the diagnosis of NONSTMI. During coronary angiography, the patient developed a dissection that started with the left main coronary artery and progressed to the circumflex and left descending artery. Good oral hygiene and dental care are more effective than antibiotic prophylaxis. The purpose of the study is to determine the patients who were diagnosed with CHD through echocardiography and to reveal the risk factors that may cause congenital heart anomalies in our center.\u0000Methods: The medical records of pediatric patients applied to our pediatric cardiology clinic between January 2010 –December 2017 were retrospectively reviewed.\u0000Results: 147 cases were diagnosed with CHD. 60 (52.6%) of the cases were female and 54 (47.4) were male. 77 of the 114 patients included in the study were acyanotic (67.5%) and 37 (32.5%) were cyanotic. Seventeen of the acyanotic patients had mixed CHD. The mean maternal gestational age was 28.2±5.78 years. 26 (22.8%) were born preterm. 17 (14.9 %) patients were born with low birth weight (under 2500 g). 8 (7%) patients were LGA (large for gestational age) (birth weight over 4000 g). Average birth weight was 2,98±740,8 gr. Consanguinity was found in the parents of 28 (24.5%) patients. There was a history of CHD in the relatives of 6 (5.2%) patients, siblings of 4 (3.5%) patients, and parents of 5 (4.3%) patients. It has frequently been claimed to be linked to right ventricular infarction, deep vein thrombosis, or pulmonary embolism. There have been cases of severe chronic obstructive pulmonary disease and profound hypoxia, which may lead to right ventricular dysfunction and thrombus in the isolated right ventricle. An 80-year-old man with acute respiratory failure was hospitalized to our emergency room. He also had Alzheimer's, COPD, diabetes mellitus, and atherosclerotic heart disease. The patient wasn't consistently using his prescription drugs and oxygen therapy. His lungs were examined, and wheezing or a drawn-out expiration was found. S1 and S2 were normal on cardiac auscultation, and there was no S3, S4, or murmur. Similar to earlier ECGs, the electrocardiogram displayed T wave inversion in leads V1-V5 and D2-D3-AVF. No pulmonary embolism was observed with computed tomography. A 1.5x2.8 cm mass was discovered between the right ventricular trabeculae during routine echocardiography, which was used to assess heart function. Moreover, there was middle tricuspid regurgitation, right ventricular dilatation, and an 8 mm tricuspid annular plane systolic extension (TAPSE). To make the diagnosis clearer, cardiac magnetic resonance imaging was carried out. Nodular forms between the trabeculations in the right ventricle were seen using cardiac resonance imaging and were considered to be thrombi because they appeared hypointense on late contrast images. Treatment with apixaban and an inhaler were both initiated. A control examination was scheduled for the patient. Transthoracic echocardiography, transesophageal echocardiography, and cardiac magnetic resonance imaging can all identify RV thrombus. Although transthoracic surgery is the procedure of choice in clinical practice, right ventricular thrombus diagnosis is not usually as straightforward as left ventricular thrombus diagnosis. According to recent studies, the most precise method for detecting myocardial thrombus is cardiac MRI. Altunbas","doi":"10.51271/jccvs-0002","DOIUrl":"https://doi.org/10.51271/jccvs-0002","url":null,"abstract":"Aims: The present study attempted to evaluate the prevalence of basal interatrial blocks (IABs), their associations with clinical parameters, and the interatrial conduction in the follow-up after a successful transcatheter aortic valve implantation (TAVI) procedure among patients with severe aortic stenosis.\u0000Methods: We retrospectively evaluated the findings of 90 patients undergoing TAVI in our center. Overall, we considered the presence and grades of IABs and P-wave durations in electrocardiograms (ECG), preoperative echocardiography (ECHO) findings (maximum and mean gradients and left atrium (LA) diameter), valve size and type, and changes in these parameters at sixth month.\u0000Results: Forty-six patients were included in the study which are suitable for the pre-determined inclusion criteria. We found the mean age of the patients to be 74.78 ± 8.66 years. In preoperatively-evaluated ECGs, while we detected partial IABs in 37% of the patients, there were advanced IABs in 6.5%, but 56.5% yielded no interatrial conduction disorder. On the other hand, in postoperatively-evaluated ECGs, while we observed partial IABs in 30.4% of the patients, there were advanced IABs among 21.7% (p = 0.017). Nevertheless, we could not conclude any IABs among 47.8% of the patients. Besides, 54.3% of the patients received a self-expandable valve, and a balloon-expandable valve was inserted in 45.7%. In this regard, we detected partial (7 patients) and advanced (2 patients) IABs in the preoperatively-evaluated ECGs of the patients receiving a self-expandable valve. In the postoperative ECGs of these patients, while the partial IAB remained the same in 4 patients (57.1%), it progressed to an advanced IAB in 3 (42.9%). In addition, while the advanced IAB regressed to a partial IAB in one patient, it remained the same for the other patient. In this group, the mean P-wave durations were found to be 118.4±22.67 before the TAVI and 119.6±21.69 after the TAVI (p = 0.113). In the preoperative ECGs of 21 patients with a balloon-expandable valve, we detected partial IABs in 10 patients and an advanced IAB in one patient. While a partial IAB developed in five patients (p = 0.022), five patients with a partial IAB developed an advanced IAB following the procedure (p = 0.022). In this group, we noticed a significant difference between preoperative (127.62±19.4) and postoperative (138.71+ 32.03) P-wave durations (p = 0.038).\u0000Conclusion: In a nutshell, we concluded no significant change in interatrial conduction time of the patients with TAVI compared to the baseline in their sixth-month ECGs. When considered by valve type, we concluded that the development and progression of IABs were significant among those with a balloon-expandable valve.
