Vacuum extraction can prevent prolonged second stage of labor, birth asphyxia in the presence of fetal distress, or when maternal pushing is contraindicated. However, vacuum-assisted births, especially those involving mid-pelvic rotational deliveries, have increasingly been replaced by cesarean sections, even though they generally carry a potentially higher risk for women and future children. This article provides detailed information on the indications and contraindications of vacuum-assisted birth, as well as the basic principles of the vacuum technique. It also includes a dedicated section on vacuum extraction for rotational births. The purpose of the present is to make a contribution to the understanding of this disease, with regard to its diagnosis and in particular the optimization of its surgical treatment. Although serious complications have been described, Amyand's hernia usually has a good prognosis. Nevertheless, practitioners must master the clinical and surgical particularities of Amyand's hernia in order to ensure optimal and adequate treatment of the hernia, while avoiding any recurrence. Questions remain as to its pathophysiology, clinical particularity and treatment. {"title":"An Unknown Complication of a Rare Combination of Intestinal Invagination and Meckel’s Diverticulum Invaginated in An Adult: A Case Report","authors":"S. Ragazzi, A. Buttafuoco, S. Invagination is responsible for only 1-3% of adult-onset intestinal obstruction. Meckel’s diverticulum, on the other hand, is the most common congenital anomaly of the gastrointestinal tract. Most of them are asymptomatic; they are recognized when they complicate, and can present with diverticulitis, gastrointestinal bleeding, intestinal obstruction, perforation, or invagination. Invagination with the Meckel’s diverticulum (MD) is a rare cause of chronic abdominal pain in the adults. We wish to present this first case of intestinal invagination and Meckel ‘s diverticulum invaginated. Presentation of case: We report the case of a 17-year-old man who was admitted to the emergency room due to a diffuse abdominal pain. Abdominal CT scan showed intestinal invagination and intestinal obstruction. The exploratory laparoscopy revealed the presence of intestinal invagination and segmental resection of the small intestine was performed. The patient was discharged on the sixteen post-operative day. Discussion: The prevalence of MD is 1 to 4%. Diagnosis is often difficult and delayed because clinical symptoms are not specific and the diagnosis is performed mainly by imaging studies. Intestinal obstruction is a more common complication in adults, whereas in children, bleeding is the more common complication. In our case, the patient had a diffuse abdominal pain with small bowel obstruction. Laparoscopy may be useful for confirming the presence of intestinal invagination, and demonstrating the underlying organic lesion serving as the lead point. Only with the laparotomy we have been able to notice the combination of intestinal invagination and MD invaginated. Pulcinelli, Andrea Maria D’armini","doi":"10.59657/2993-1126.brs.22.002","DOIUrl":"https://doi.org/10.59657/2993-1126.brs.22.002","url":null,"abstract":"We report a peculiar case of a 77 years woman affected by chronic thromboembolic pulmonary hypertension (CTEPH) with surgical indication to pulmonary endarterectomy (PEA) but with the concomitant presence of anti-PF4-heparin autoantibodies. Coagulation disorders are frequently associated in patients with CTEPH and therefore full thrombophilic pattern evaluation is completed as standard preoperative protocol in all our patients. In this case, history of chronic thrombocytopenia induced us to extend preoperative evaluation to the presence of anti-PF4-heparin autoantibodies. Once heparin-induced thrombocytopenia was documented, an alternative approach rather than standard cardiopulmonary bypass with systemic heparinization was mandatory. Due to the characteristics of surgical procedure (i.e., prolonged CPB time, possible risk of intraoperative and postoperative bleeding), we decided to adopt plasmapheresis and immunoglobulin infusion before surgery, thus allowing the standard use of heparin in CPB. The patient underwent a successful PEA without complications.
