{"title":"Use of transparent cover and elastic fixation band in patients with peripheral venous catheter: a comparison of microbiological colonisation.","authors":"Busra Nur Orhan, Sonay Goktas","doi":"10.12968/jowc.2022.0057","DOIUrl":"https://doi.org/10.12968/jowc.2022.0057","url":null,"abstract":"Objective: The study was carried out to compare the effect of transparent covers and elastic fixation bands used in peripheral venous catheter fixation on microbiological colonisation in patients in intensive care.Method: This experimental randomised controlled study was conducted with patients treated in the Anesthesia and Reanimation Unit (an intensive care unit, ICU) of a training and research hospital in Istanbul between January 2019 and May 2020. Participants were divided into two equal groups: intervention: (catheters fixed with a transparent cover) and control (catheters fixed with an elastic fixation band) by the block randomisation method (CONSORT). After 72 hours, the catheters were removed aseptically and cultures were collected from the catheter tips. The roll plate technique was used and colony growth of ≥15 colony forming units was considered significant. The data of the study were collected using the 'Patient Information Form' and 'Catheter Follow-up and Result Form'.Results: A total of 60 patients were included in the study (30 in each group). The Clinical Frailty Scale (CFS) was used to measure frailty at the patients' initial appointment. Based on their CFS scores, the patients were divided into two groups: non-frail patients (with a level of 1-4); and frail patients (with a level of ≥5).Results: The median age of the 97 patients included in the study was 56 years, and median follow-up period was 3.8 months. Based on their CFS levels, 36 (37.1%) patients were defined as frail. There were considerably more patients in the frail group with Wagner stages 3 and 4 DF (p<0.001). More than half of patients in the frail group required a minor amputation for their DF (52.7% compared with 24.6% in the non-frail group; p=0.005).Conclusion: DF is a debilitating disease which can lead to frailty, even in the non-geriatric population. A phenomenological approach of qualitative research was used, and the data were collected and analysed using Colaizzi's method.Results: The research cohort comprised 15 patients and three themes were extracted: (1) symptom management burden (symptom distress and lack of knowledge about wound care); (2) inadequate resources related to self-management (insufficient medical resources in primary hospitals, tortuosity of the treatment-seeking process, expectation of access to wound expertise); and (3) self-managing pressures coexisting with motivation (high expectations for and low level of attention to wound healing, varied treatment adherence, financial burden, negative emotions, the contradiction between work and rest, and social communication impact).Conclusion: There was variation in the perception of self-management among patients with hard-to-heal wounds, with only some patients knowing how to perform wound self-management at home. Overall, patients with hard-to-heal wounds had poor perceptions of self-management. Healthcare workers should encourage and guide patients with hard-to-heal wounds to self-manage, which will help to increase the patient's motivation for self-management and reduce the burden of symptom management. Methods to facilitate wound healing have become a focus for treatment, with added benefits of reducing pain and improving prognosis.Method: In recent years, high-intensity laser therapy (HILT) has generated substantial interest within the fields of basic research and clinical rehabilitation. In particular, the use of HILT has been shown to have benefits in healing perineal wounds after eight weeks of treatment. This study investigated the effects of HILT on accelerating the healing of a postoperative perineal wound.Case presentation: The case of a 65-year-old man who had undergone abdominoperineal resection for rectal cancer is presented. This study compared the effectiveness of a cross-localisation method with a direct photographic method for image acquisition of breast cancer wounds.Method: The wounds of eligible patients were assessed by direct measurement, direct photography, and by a cross-localisation method using the Bates-Jensen Wound Assessment Tool (BWAT) with 13 items. After four months, the wound images were re-evaluated and re-recorded for consistency assessment.Results: The experimental cohort comprised 150 patients. The consistency of the total BWAT score between the direct photograph acquisition and direct measurement methods was poor (Kappa=0.375). Only wound size, induration and exudate type presented general consistency. There was a good consistency between the cross-localisation image method and the direct measurement method (Kappa=0.786). The wound size, depth, oedema, induration and granulation tissue demonstrated complete consistency between the direct measurement and cross-localisation methods (Kappa=1.000).Conclusion: Wound imaging using direct photography does not present the wound characteristics with complete clarity; however cross-localisation with an objective reference showed optimal performance in this study.
