The impact of EBV status on characteristics and outcomes of central nervous system lymphoproliferative disorder
Central nervous system lymphoproliferative disorder (CNS-LPD) is a rare disease of the central nervous system (CNS) with significant misdiagnosis and mortality, and Both EBV-positive (EBVpos) and EBV-negative (EBVneg) cases have been reported. To investigate the relationship between EBV status and the clinical characteristics and outcomes of CNS-LPD patients, we conducted a retrospective cohort study with 39 EBVpos and 41 EBVneg patients with CNS-LPD. EBVpos patients had higher rates of cerebrospinal fluid (CSF) leukocytosis, elevated protein levels, and worse functional outcomes, and no significant difference in survival was found between EBVpos and EBVneg patients. Regression analysis identified ECOG performance status ≥2 as an independent risk factor for overall survival and age ≥60, EBV-positivity, and ECOG performance status ≥2 as independent risk factors for poorer functional prognosis. These findings highlight that different management strategies should be developed for different EBV status patients.
