Traditional reconstruction techniques, including muscle flaps and dermal-fat grafts, are associated with significant limitations. This technical report explores the innovative use of the buccal fat pad (BFP) as a vascularized graft for restoring facial symmetry and reducing Frey’s syndrome risk. Its proximity to the surgical site, consistent </span>vascularity<span>, and minimal donor morbidity make the BFP an ideal reconstructive material. Surgical application in two patients with accessory lobe tumors demonstrated effective restoration of facial contours and defect coverage without complications.
