The seventh cranial nerve palsy occurred in 9% of Lyme disease cases and the majority of them present as unilateral facial palsy. We present a rare case of bilateral facial palsy in Lyme disease due to <i>Borrelia burgdorferi</i> infection. A total of eleven cases of Lyme disease with bilateral facial palsy reported in literature were summarized and compared to our case.
