Histopathological examination revealed mild spongiosis with perivascular neutrophilic and mononuclear cell infiltrate, confirming the diagnosis of PP. Treatment with oral doxycycline and discontinuation of the ketogenic diet led to lesion resolution within one month, with subsequent postinflammatory hyperpigmentation.
