Abstract
The first quarter of the 21st century has seen critical advances in the conduct and reporting of clinical trials in all fields of medicine. Beyond the foundational requirement for prospective registration, the widespread adoption of the intention-to-treat principle, appropriate treatment of missing data, and a focus on the clinical relevance of outcomes have predominated. However, there is growing awareness of the need for further refinements in many aspects of our trials.
