Sacral giant chronic expanding hematoma in a patient with Marfan syndrome: A case report
Background
Chronic expanding hematoma (CEH) is defined as a hematoma that gradually expands over more than one month following trauma or surgery. This report describes a rare case of giant CEH developed in the sacrum spreading toward the retroperitoneal space.
Case presentation
A 62-year-old female with Marfan syndrome complained of low back pain and urinary/fecal incontinence. She had surgeries for aortic aneurysm, and an anticoagulant treatment. At the age of 49 and 54, she had partial resection of a mass lesion in the sacrum extending to the retroperitoneal space. Another surgery was planned for the recurrent, giant mass lesion which was confirmed as CEH by a needle biopsy. The resection of CEH via combined posterior and anterior approach relieved her symptoms for two years after surgery.
Conclusion
CEH can develop as a massive mass lesion in the sacrum. Anticoagulants and Marfan syndrome could have negative effects on CEH.
