LAT, 3 mots simples pour des réalités complexes

Les limitations et arrêts de traitements (les « LAT ») renvoient à une pratique centrale de l’éthique du soin palliatif pédiatrique avec de forts enjeux décisionnels. Le caractère polysémique de cet acronyme nous a conduit en premier lieu à esquisser la question du sens des LAT et à explorer ce qui en découle : les significations et représentations qui y sont associées, la direction que prend le soin, les temporalités brusquées à l’approche de la décision de LAT, le contexte, la manière dont les LAT peuvent faire sens pour les soignants, engagés pour chacun à une place singulière auprès de l’enfant et de sa famille. Le dispositif des LAT pensé par la loi vient borner la mise en jeu des effets et affects subjectifs et constitue un appui précieux pour les équipes. Outil structurant, pondérateur des subjectivités, autant de fonctions pour continuer à penser et à viser une décision la plus juste possible. Pour autant, au-delà des fonctions identifiées ou identifiables des LAT, n’y aurait-il pas dans les « arrière-pensées » latentes de nos intentions d’objectivité et de justesse des sens cachés, des intentions sous-jacentes à cette pratique clinique ? Y aurait-il de l’inavouable dans les « angles morts » des LAT ? Quand on « fait des LAT », les émotions et l’angoisse suscitées chez les soignants font partie de la donne. L’utilité de la portée subjective agit comme garant d’un processus de « LAT sensées » et empreintes d’humanité.

Treatment withholding or withdrawal (LAT in French) refers to a central practice in pediatric palliative care ethics with high decisional stakes. The polysemous nature of this acronym invites (or has led us) first to outline the question of the meaning of LATs and to explore what follows from it: the meanings and representations associated with it, the direction that care takes, the temporalities that are abrupted as the LAT decision is approached, the context, and the way in which LATs can make sense for caregivers, each of whom is committed to a singular place with the child and his/her family. The LAT system designed by the law limits the effects and subjective affects and constitutes a precious support for the teams. It is a structuring tool, a balancing of subjectivities, all of which help to continue to think and to aim for the fairest possible decision. However, beyond the identified or identifiable functions of LATs, might there not be hidden meanings and intentions underlying this clinical practice in the latent “ulterior motives” of our intentions of objectivity and accuracy ? Could there be something unmentionable in the “blind spots” of LATs? When “doing LATs”, the emotions and anxiety aroused in caregivers are part of the deal. The subjective scope also acts as a guarantor of a humane and meaningful “LAT” process.
