The donor site was closed directly.</p><p><strong>Results: </strong>The size of TAAP flap is from 7.0 cm×5.0 cm to 9.5 cmu×6.0 cm. The length of pedicle is 7.5-9.0 cm. The distance from pivot point of flap to central point of recipient site was 9.0-10.5 cm. All 13 flaps survived completely, the defect at donor site was closed directly in all cases. 摘要

目的:探讨胸肩峰动脉穿支皮瓣修复下咽缺损的临床效果。方法:2011年6月至2015年11月,对13例下咽癌复发患者采用TAAP皮瓣修复肿瘤性下咽缺损。下咽缺损大小为7.0 cm× 4.5 cm ~ 8.5 cm× 5.5 cm。应用多普勒检测胸肩峰动脉穿支位置。皮瓣设计在肩峰与剑突连接线上,尺寸比缺损大0.5 ~ 1.0 cm,经胸大肌锁骨头下方,锁骨与胸锁乳突肌下方至受术者部位植入。供体部位被直接关闭。结果:TAAP皮瓣大小为7.0 cm×5.0 ~ 9.5 cmu×6.0 cm。蒂长7.5-9.0 cm。皮瓣枢轴点至受体中心点的距离为9.0 ~ 10.5 cm。13例皮瓣全部成活,供区缺损均直接闭合。随访8 ~ 42个月,外观良好,无复发、瘘、狭窄/狭窄、裂裂、肿胀,供体部位仅留下线状疤痕,所有患者胸大肌功能完全保留。结论:TAAP皮瓣是下咽缺损修复的理想选择。
