As part of the journal's 30th anniversary, this article presents a critical reflection on the advances, challenges, and emerging scenarios in Workers' Health (WH), based on its publications. Articles published between 1998 and 2024 were analyzed using data from SciELO and C&SC archives. In this period, 292 articles on WH were published, of which 171 were empirical studies focused on specific occupational groups, with particular emphasis on industrial workers (14%), healthcare workers (12.9%), and rural workers (9.9%). The publications featured diverse theoretical and methodological approaches, with a predominance of qualitative studies (35.9%), epidemiological studies 32.7%), and studies on WH policies (17.1%). A lack of research on emerging topics such as remote work and algorithmic management was noted, along with the invisibility of groups such as quilombolas, Indigenous peoples, domestic workers, and LGBTQIA+ workers. These gaps highlight ethnic-racial, gender, and epistemological inequalities, reflected in the underrepresentation of specific topics and groups.
