Higher risk of subsidence was associated with age > 50 years and preoperative Cobb angle > 50°. Quality of life improved across all measures: ODI decreased from 45% to 32%, SF-36 increased from 50 to 65, and SRS-22 declined slightly from 3.9 to 3.7 in patients with tether settling.</p><p><strong>Conclusion: </strong>ASC demonstrates high effectiveness and acceptable safety for scoliosis correction in patients over 40 years.
