摘要

近年来,法洛四联症修复后的晚期肺动脉瓣返流逐渐引起了国内外学者的关注。早期肺动脉瓣返流可能无症状,甚至存活很长时间无症状。然而,长期肺动脉瓣返流和慢性右心室容量过载可导致运动耐量降低、右心室射血分数降低、心律失常和猝死。如何为长期肺动脉瓣返流的修复性法洛四联症患者提供规范化、科学化、个性化的治疗策略和全生命周期健康服务,已成为世界各国冠心病外科医生面临的挑战。本文就法洛四联症修复后肺动脉瓣反流的病理生理机制、肺瓣反流严重程度的术前诊断及评价、预防法洛四联症在肺动脉瓣反流前的初步修复、严重肺动脉瓣反流干预时间的确定、最佳干预策略的选择等方面进行综述。展望组织工程瓣膜导管的临床应用前景,有助于改善修复后法洛四联症患者的远期预后和生活质量。
