Among high-risk anatomic features, totally anomalous origin of the coronary arteries from the pulmonary trunk is extremely rare, with only 57 cases reported in the literature as to our knowledge. We report a case of such a congenital anomaly with a successful surgical repair and good cardiac function recovery.
