We report the case of a 44-year-old man presenting with a left scrotal mass managed with fertility-sparing surgery and adjuvant therapy. Pathological analysis confirmed grade II leiomyosarcoma. Multidisciplinary management emphasized the balance between oncological control and fertility preservation.
