The value of log-2 mEASIX before conditioning (-7 d) was calculated, and the patients were divided into a low-mEASIX group (42 patients) and a high-mEASIX group (28 patients) based on the cut-off value of 5.443 determined by the receiver operating characteristic (ROC) curve. Eventually, the predictive role of mEASIX before conditioning on the efficacy of CAR-T cell therapy and CRS was analyzed.</p><p><strong>Results: </strong>The high-mEASIX group exhibited significantly worse median overall survival (OS) and median progression-free survival (PFS) in comparison to the low mEASIX group (OS: 3.2 months <i>vs</i> not reached, <i>P</i> < 0.01; PFS: 1.3 months <i>vs</i> 6.0 months, <i>P</i> =0.009). The incidence of grade ≥2 CRS in the high-mEASIX group was substantially higher than that in the low-mEASIX group (57.1% <i>vs</i> 19.0%, <i>P</i> =0.007). 目的:探讨改良内皮活化和应激指数(mEASIX)对嵌合抗原受体t细胞(CAR-T)治疗疗效和细胞因子释放综合征(CRS)的预测作用。方法:回顾性分析徐州医科大学附属医院血液科2018年9月1日至2023年2月28日接受CAR-T治疗的70例复发难治性(R/R) b细胞肿瘤患者的临床资料。计算调节前(-7 d)的log-2 mEASIX值,并根据受试者工作特征(ROC)曲线确定的截断值5.443将患者分为低mEASIX组(42例)和高mEASIX组(28例)。最后分析调理前mEASIX对CAR-T细胞治疗疗效和CRS的预测作用。结果:与低mEASIX组相比,高mEASIX组的中位总生存期(OS)和中位无进展生存期(PFS)明显较差(OS: 3.2个月vs未达到,P < 0.01; PFS: 1.3个月vs 6.0个月,P =0.009)。高measix组≥2级CRS发生率显著高于低measix组(57.1% vs 19.0%, P =0.007)。CAR-T治疗后缓解程度(P =0.001)、是否发生CRS (P =0.041)、调理前乳酸脱氢酶(LDH)水平(P =0.046)、调理前mEASIX评分(P =0.047)是影响CAR-T细胞治疗患者OS的独立因素。结论:调理前的mEASIX评分可以预测接受CAR-T细胞治疗的复发难治性b细胞肿瘤患者的OS和≥2级CRS的发生率。
