Peripheral blood mononuclear cells (PBMC) of children with ITP and healthy children were separated, and the plasma levels of M1 macrophage-associated cytokines (TNF-α, IL-6), M2 macrophage-associated cytokines (IL-10, TGF-β), and CSF-1 were detected by ELISA in the children of both groups. The mRNA levels of M1 macrophage surface markers (CD86, iNOS), M2 macrophage surface markers (CD206, Arg-1) and CSF-1R were detected by RT-PCR in PBMC of children in both groups. Western blot was used to detect the expression of CSF-1R protein in PBMC of the two groups of children. The correlation between platelet count and CSF-1R mRNA expression in PBMC, TNF-α, IL-6, IL-10, TGF-β and CSF-1 in plasma was analyzed.</p><p><strong>Results: </strong>Compared with the control group, the levels of IL-10, TGF-β, CSF-1 and platelet count in plasma of children with ITP were significantly decreased (<i>P</i> < 0.01), and the levels of TNF-α and IL-6 were significantly increased (<i>P</i> < 0.01); the mRNA levels of the M1 macrophage surface markers (CD86, iNOS) in PBMC of children with ITP were significantly increased (<i>P</i> < 0.05), mRNA levels of M2 macrophage surface marker CD206 in PBMC of children with ITP were decreased compared with controls but the difference was not statistically significant ( <i>P</i> >0.05), mRNA levels of Arg-1 were decreased, the difference was statistically significant (<i>P</i> < 0.05). The mRNA and protein levels of CSF-1R in PBMC of ITP children were higher than that in controls. 摘要

目的:探讨CSF-1和CSF-1R在免疫性血小板减少症(ITP)患儿外周血中的表达及其临床意义。方法:选取2023年2月至2024年1月在我院就诊的ITP患儿44例作为观察组,同期选取健康儿童40例作为对照组,收集相关临床资料。分离ITP患儿和健康患儿外周血单个核细胞(PBMC), ELISA检测两组患儿血浆M1巨噬细胞相关因子(TNF-α、IL-6)、M2巨噬细胞相关因子(IL-10、TGF-β)、CSF-1水平。RT-PCR检测两组患儿PBMC中M1巨噬细胞表面标志物(CD86、iNOS)、M2巨噬细胞表面标志物(CD206、Arg-1)和CSF-1R mRNA水平。Western blot检测两组患儿PBMC中CSF-1R蛋白的表达。分析血小板计数与PBMC中CSF-1R mRNA、TNF-α、IL-6、IL-10、TGF-β、CSF-1表达的相关性。结果:与对照组比较,ITP患儿血浆中IL-10、TGF-β、CSF-1水平及血小板计数均显著降低(P < 0.01), TNF-α、IL-6水平显著升高(P < 0.01);ITP患儿PBMC中M1巨噬细胞表面标志物(CD86、iNOS) mRNA水平显著升高(P < 0.05), M2巨噬细胞表面标志物CD206 mRNA水平较对照组降低但差异无统计学意义(P < 0.05), Arg-1 mRNA水平降低,差异有统计学意义(P < 0.05)。ITP患儿PBMC中CSF-1R mRNA和蛋白水平均高于对照组。ITP PBMC中CSF-1R的表达与血小板计数、IL-10、CSF-1呈正相关(r =0.822、0.481、0.405)。结论:ITP血浆中CSF-1显著降低,ITP PBMC中CSF-1R mRNA和蛋白表达显著升高,参与调节巨噬细胞M1/M2失衡,可能是ITP潜在的治疗靶点。
