This study compared the effect of the pressure massage of Hegu point with Kunlun-Taixi points on the pain of intramuscular injection in premature newborns.</p><p><strong>Materials and methods: </strong>This triple-blind, parallel, clinical trial was conducted in the Neonatal Intensive Care Unit (NICU) of Shahid Rahimi Hospital, Khorramabad, Iran. Participants included 100 premature newborns randomly assigned to four groups (<i>n</i> = 25). The intervention groups received a counterclockwise pressure massage of the Hegu or Kunlun-Taixi points for two minutes. The placebo group received a pressure massage of the tip of the nose. The control group received routine care. The Astrid Lindgren and Lund Children's Hospital Pain Scale was used to observe pain behaviors before, during, and after intramuscular injection of vitamin K. One-way Analysis of Variance (ANOVA), repeated measures ANOVA, and Chi-square tests were used for data analysis.</p><p><strong>Results: </strong>The mean (Standard Deviation) of pain scores during injection in the Hegu massage, Kunlun-Taixi massage, control, and placebo groups were 5.92 (1.99), 5.94 (1.75), 6.66 (1.45), and 6.56 (1.61), respectively. Despite the lower pain scores of the intervention groups compared to the control and placebo groups, their difference was not statistically significant (<i>F</i> = 1.32; <i>p</i> = 0.27).</p><p><strong>Conclusions: </strong>Using pressure massage before intramuscular injection of vitamin K has no significant effect on intramuscular injection pain in premature newborns. 背景:在早产新生儿中进行疼痛手术是很普遍的。本研究比较了压揉合谷穴与昆仑太西穴对早产儿肌注疼痛的影响。材料和方法:这项三盲、平行临床试验在伊朗霍拉马巴德Shahid Rahimi医院新生儿重症监护病房(NICU)进行。参与者包括100名早产儿,随机分为四组(n = 25)。干预组按合谷穴或昆仑太西穴逆时针按压2分钟。安慰剂组接受鼻尖的压力按摩。对照组接受常规护理。采用Astrid Lindgren and Lund儿童医院疼痛量表,观察肌肉注射维生素k前、中、后的疼痛行为。数据分析采用单因素方差分析(ANOVA)、重复测量方差分析(repeated measures ANOVA)和卡方检验。结果:合谷按摩组、昆仑太西按摩组、对照组、安慰剂组注射时疼痛评分均值(标准差)分别为5.92(1.99)、5.94(1.75)、6.66(1.45)、6.56(1.61)。尽管干预组的疼痛评分低于对照组和安慰剂组,但差异无统计学意义(F = 1.32; p = 0.27)。结论:维生素K肌内注射前按压按摩对早产儿肌内注射疼痛无显著影响。我们建议,未来的研究应该进行更长的时间的按摩,以及其他缓解早产儿疼痛的方法。
