Among them, 109 patients received TTE (TTE group), and another 95 received TEE (TEE group). Differences in the evaluation effects of preoperative echocardiography and multi-slice helical computed tomography (MSCT) in all patients were compared, and changes in echocardiographic parameters before and after surgery were observed. In addition, the differences in postoperative hemodynamics, cardiac function [brain natriuretic peptide (BNP), cardiac troponin I (cTnI), creatine kinase isoenzyme (CK-MB)], stress response [superoxide dismutase (SOD), malondialdehyde (MDA)], inflammatory factors [Interleukin-1b/6 (IL-1b/6), tumour necrosis factor-a (TNF-a)], and pain mediators [5-hydroxytryptamine (5-HT), endothelin-1 (ET-1), prostaglandin E2 (PGE2), substance P (SP)] between the observation and TTE groups were compared.</p><p><strong>Results: </strong>No differences were identified in the evaluation of the aortic root between echocardiography and MSCT (P>0.05). After surgery, parameters such as LVESD and IVST decreased, while LVEF and AVA increased (P<0.05). The TEE group showed superior postoperative hemodynamics to the TTE group (P<0.05). 背景:我们比较了经胸超声心动图(TTE)和经食管超声心动图(TEE)对严重主动脉瓣狭窄(AS)患者血流动力学、炎症应激反应和疼痛介质的影响。方法:选择2022年1月~ 2024年2月在我院行经导管主动脉瓣置换术(TAVR)治疗的重度AS患者204例为研究对象。其中,TTE组109例,TEE组95例。比较所有患者术前超声心动图和多层螺旋ct (MSCT)评价效果的差异,观察术前、术后超声心动图参数的变化。并比较观察组与TTE组术后血流动力学、心功能[脑利钠肽(BNP)、心肌肌钙蛋白I (cTnI)、肌酸激酶同工酶(CK-MB)]、应激反应[超氧化物歧化酶(SOD)、丙二醛(MDA)]、炎症因子[白细胞介素-1b/6 (IL-1b/6)、肿瘤坏死因子-a (TNF-a)]、疼痛介质[5-羟色胺(5-HT)、内皮素-1 (ET-1)、前列腺素E2 (PGE2)、P物质(SP)]的差异。结果:超声心动图与MSCT对主动脉根的评价无差异(P < 0.05)。术后LVESD、IVST等指标降低,LVEF、AVA升高(p < 0.05),但TEE组IL-1b、IL-6、TNF-a、5-HT、ET-1、SP低于TTE组,SOD高于TTE组(p < 0.05)。结论:TTE和TEE对重度as患者实施TAVR有很好的指导作用,其中TEE更有助于提高TAVR的有效性和安全性。
