However, no previous literature documents Sigmoidovesical fistula (SVF) expansion with sepsis following transurethral resection of prostate (TURP).</p><p><strong>Case presentation: </strong>We report a 73-year-old male with benign prostatic hyperplasia who developed SVF expansion and postoperative sepsis following TURP. Emergency transverse colostomy achieved infection control, followed by successful laparoscopic sigmoidectomy during secondary surgery.</p><p><strong>Conclusion: </strong>This case highlights the need to remain vigilant for occult SVF risks during TURP procedures.
