Collision tumors are relatively rare, and intracranial collision tumors are even more uncommon. Reports of pituitary adenoma combined with craniopharyngioma are exceedingly scarce.</p><p><strong>Patient concerns: </strong>This article presents a case of a 75-year-old male patient diagnosed with a collision tumor consisting of a pituitary adenoma and craniopharyngioma in the sellar region.</p><p><strong>Diagnosis: </strong>After hospital admission, MRI examination revealed a mass in the sellar and suprasellar region, suggestive of a pituitary adenoma combined with a craniopharyngioma. The patient underwent trans-sphenoidal endoscopic tumor resection. Intraoperatively, a mass was observed in the sellar and suprasellar regions. Postoperative pathology confirmed a diagnosis of pituitary adenoma combined with craniopharyngioma.</p><p><strong>Interventions: </strong>The patient underwent endoscopic transnasal surgery, successfully resecting the tumor. Postoperatively, the patient received hormone replacement therapy and was regularly monitored.</p><p><strong>Outcomes: </strong>Postoperative MRI indicated that the majority of the tumor had been resected. The patient developed hormonal imbalances post-surgery, which improved after hormone replacement therapy. The patient's condition stabilized, was cured, and was dismissed from the hospital..</p><p><strong>Lessons: </strong>Early detection and timely surgical intervention remain the optimal management strategies.
