Current approaches to dementia care are often fragmented, marked by poor coordination and limited communication among healthcare providers. This lack of integration contributes to delayed diagnoses, inadequate treatment, and heightened caregiver stress. This article emphasizes the critical role of inter-professional collaboration (IPC) in addressing these challenges and improving outcomes for people living with dementia (PLWD). Effective IPC involves coordinated efforts among physicians, nurse practitioners, nurses, home health aides, therapists, and social workers to deliver comprehensive, patient-centered care that meets the complex cognitive, physical, and psychosocial needs of PLWD.
