This narrative review outlines the pathophysiology, screening strategies, and evolving diagnostic tools such as noninvasive genotyping and MCA Doppler. We examine immunomodulatory therapies like IVIg and nipocalimab, and discuss technical considerations for intrauterine transfusions. Finally, we address neonatal management and long-term outcomes.
