A 15-year-old male presented with orbital cellulitis, acute sinusitis, and meningismus. Brain imaging showed evidence of early intracranial abscess formation and areas of cerebral infarction. He underwent urgent endoscopic sinus surgery (ESS) and drainage of an orbital subperiosteal abscess.
