We report a case of a 69-year-old man with end-stage chronic obstructive pulmonary disease and obesity (body mass index 35.55 kg/m<sup>2</sup>) declined for lung transplantation. Six months post-ESG, the patient's body mass index decreased to 30.17 kg/m<sup>2</sup>, with significant improvements in exercise capacity, oxygen requirements, and pulmonary function. Consequently, transplantation was deferred due to improved clinical status.
