Here, we presented a 41-year-old man with orbital-nasal alveolar rhabdomyosarcoma who underwent both 18 F-FDG and 18 FAl-NOTA-FAPI PET/CT. 18 FAl-NOTA-FAPI PET/CT showed intense uptake in the primary lesion and regional metastatic lymph nodes with an excellent tumor-to-background ratio, compared with the mild to moderate uptake on 18 F-FDG PET/CT. In addition, 18 FAl-NOTA-FAPI PET/CT detected bone metastases that were not identified by 18 F-FDG PET/CT, which altered the patient's tumor stage and treatment strategy.
