Digital rectal examination revealed a mass lesion in the lower rectum. The patient was referred to our hospital and finally diagnosed with cT4a, N1b, M0, and c-Stage Ⅲb locally advanced rectal cancer. Neoadjuvant chemotherapy(NAC)with CAPOX and bevacizumab was administered to facilitate R0 resection, permitting anal preservation. When the chemotherapeutic regimen was evaluated to have achieved a complete clinical response, the patient underwent laparoscopic low anterior resection. He was shortly discharged without complications. No residual cancer was revealed upon histopathological examination, demonstrating a complete pathological response. Although NAC for rectal cancer has not been recommended by the Japanese guidelines, high response rates have recently been reported.
