The peritoneal nodule macroscopically comprised a fibro-fatty piece of tissue and histologically demonstrated mature spermatozoa with surrounding histiocytic reaction. A diagnosis of sperm granuloma was made. Sperm granuloma typically occurs in the epididymis following damage to vas deferens.
