We present 68 Ga-FAPI-04 findings of multiple lymph node metastases of gastric hepatoid adenocarcinoma in a 60-year-old man. In this case, 68 Ga-FAPI PET/CT showed increased FAPI activity in the primary gastric lesions and multiple lymph nodes throughout the body.
