Women accounted for 29% of first authors and 23% of corresponding authors, with a significant increase in female authorship over time. However, overall representation remains disproportionately low.</p><p><strong>Discussion: </strong>Our study highlights the persistent underrepresentation of women in otology and neurotology research, despite their growing presence in medicine.
