A stroke of bad luck: HSV-1 encephalitis in the aftermath of a cerebrovascular accident
Herpes simplex virus type 1 (HSV-1) encephalitis is a serious and often fatal condition caused by inflammation of the brain parenchyma. We present a case of a 48-year-old male with a complex medical history who presented with symptoms of right arm weakness, dysarthria, and right facial droop suggestive of an acute cerebrovascular accident. His clinical course was marked by persistent fever and altered mental status. Lumbar puncture revealed a diagnosis of HSV-1 encephalitis. This case focuses on a unique situation in which a cerebrovascular accident and HSV-1 encephalitis manifested together. In our case, MRI showed infarcts in the globus pallidus, which goes against HSV-induced infarcts as HSV spares basal ganglia.
