While patients with GA are generally asymptomatic, nonspecific abdominal pain localized to the right upper quadrant and epigastrium is the most frequently reported symptom. We herein present a case of diffuse gallbladder adenomyomatosis that presented as acalculous cholecystitis, characterized by right upper quadrant pain, fever, and elevated inflammatory markers.
