A total of 60 patients with ischemic stroke at Hebei University Hospital were selected from October 2020 to September 2022 and randomly divided observation group and control group. The control group was given conventional rehabilitation therapy, with the observation group receiving digital treadmill training. Both groups were assessed using the Holden Functional Ambulation Category Scale(FAC), digital treadmill gait analysis system, and dynamic balance instrument and after four weeks, followed by a statistical analysis.</p><p><strong>Result: </strong>After four weeks of treatment, both groups showed an improved in FAC grading, gait spatiotemporal parameters (including step speed, step length, stance time and step height), gait kinematic parameters (including hip joint flexion and extension mobility, knee joint flexion and extension mobility, and active ankle dorsiflexion ability) and balance assessment parameters (including motion trajectory area and motion trajectory perimeter) compared to pre-treatment (p< 0.05). Additionally, the gait symmetry parameters, including step length deviation and step height deviation, in the observation group were significantly lower than pre-treatment (p< 0.05), while in the control group, without statistically significant differences(p>0.05). 摘要

目的:观察数字跑步机对缺血性脑卒中患者下肢运动功能的影响。方法:采用临床比较研究。选取2020年10月~ 2022年9月河北大学附属医院缺血性脑卒中患者60例,随机分为观察组和对照组。对照组给予常规康复治疗,观察组给予数字跑步机训练。两组均采用霍顿功能行走分类量表(FAC)、数字跑步机步态分析系统和动态平衡仪进行评估,四周后进行统计分析。结果:治疗4周后,两组患者FAC评分、步态时空参数(步速、步长、站立时间、步高)、步态运动学参数(髋关节屈伸活动度、膝关节屈伸活动度、踝关节主动背伸能力)、平衡评估参数(运动轨迹面积、运动轨迹周长)均较治疗前改善(p< 0.05)。观察组步态对称参数步长偏差、步高偏差均显著低于治疗前(p< 0.05),对照组差异无统计学意义(p< 0.05)。组间比较,观察组以上各项指标较对照组均有显著改善(p< 0.05)。结论:数字跑步机训练可有效改善缺血性脑卒中患者的步态参数、步态对称性、行走和平衡能力。
