They have a high risk of malignant transformation, due to sun exposure or chronic trauma. We present a male patient, 42 years old, who comes for dental treatment. During the oral and facial evaluation, a raised lesion is observed in the frontal region to the right of the midline, with a slight increase in pigmentation, and slight discomfort upon exposure. Solar, at the patient's request, the excisional biopsy was performed and sent for histopathological analysis, the result was a pigmented intradermal melanocytic nevus with significant activity in the dermo-epidermal junction where atypical melanocytes arranged in isolation were noted.
