And three themes were identified: (1) challenges of transition and rehabilitation, (2) adaptation and coping, (3) ensuring patient-centered rehabilitation and discharge care.</div></div><div><h3>Conclusion</h3><div>It is crucial for healthcare staff to address the discharge transition experiences and nursing needs of elderly patients with hip fracture. Background
