摘要

背景:心脏手术相关急性肾损伤是心脏手术后常见的严重并发症。目前尚无专门的药物治疗方法。我们对其病理生理学的了解仍处于初步阶段。方法:共纳入2504例心脏手术后急性肾损伤(AKI)患者。采用高效液相色谱联用质谱法对血浆中代谢物进行非靶向分析,鉴定出显著差异代谢物。随后,采用基于串联液相色谱-质谱的方法,使用同位素标记的标准添加对代谢标记物n -乙酰-色氨酸进行了靶向分析。采用不同肾损伤小鼠模型和上皮细胞模型测定n -乙酰色氨酸的功能。采用转录组测序、表面等离子体共振和蛋白突变等方法探讨n -乙酰色氨酸对肾脏的作用机制。结果:基于1042例患者的队列研究,我们共鉴定出32种与AKI发生相关的差异代谢物。其中,与心脏手术后未发生AKI的患者相比,心脏手术相关急性肾损伤患者血浆中n -乙酰色氨酸升高。血浆中n -乙酰色氨酸水平较高,通过精确的靶向定量证实。n-乙酰色氨酸在缺血/再灌注、顺铂和单侧输尿管梗阻肾损伤小鼠模型中表现出肾脏保护作用。从机制上讲,n -乙酰色氨酸通过在483和508个位点与KEAP1相互作用,导致Nrf2核易位和蛋白酶体基因转录,发挥肾脏保护作用。结论:n -乙酰色氨酸在肾保护中起关键作用。
